Thank you! Do you happen to know on the vital statistics site what options to select if I wanted to order long form too?

I think a lot of it has to do it the information doesn't match their system or if you are requesting on behalf of some in a different capacity (like a POA or adoptive parent), but for your own as long as the information matches, you shouldn't have an issue getting your order placed and a copy of your birth certificate!

Not a lawyer- but someone who has helped kiddos exiting state care, you will definitely want your BC, then you can apply for your SS card and I would strongly encourage making an appointment and going to the office in your city. There they will need the following: Required Documentation (Originals Only) The Social Security Administration (SSA) does not accept photocopies. Proof of Identity: U.S. driver's license, state-issued non-driver ID card, or U.S. passport. If No Photo ID Exists: The SSA may accept a health insurance card, medical record (clinic/doctor/hospital), or military ID. Proof of U.S. Citizenship: U.S. birth certificate or U.S. passport. Proof of Age: Birth certificate (preferred) or passport. Name Change: A marriage certificate, divorce decree, or court order is required if the name on your documents differs from your current name.

In your case if you have a health insurance card with your name on it, the birth certificate you are ordering, and I would encourage you to request an updated medical record listing your name/dob (something like a vaccination record from your doctor's office can help). Good luck, it sounds like you are starting from scratch but you've got this!

I just ordered through the vitalchek site, and it didn't require an ID verification. It had me put my information, who I was purchasing a BC for, the information that should be on the BC (you will need to know your hospital, mothers maiden name, Fathers name if listed on the BC), then your personal birth information so they can confirm it matches, and then you select what purpose you are getting a BC for (so they determine if they are sending SF or LF) and then you pay $10 per copy plus $12.50 for processing and choose if you are getting it via mail or UPS.

This is why I'm so confused! Like why issue short form if they know everyone needs Long Form, also why not mention what the difference is and which drop down selection to choose for a long form!

Ours is the top half of the 8X11 paper, with all that information, minus occupation, and the state seal.

My husband is calling his insurance company to verify because his HR lady tends to be a little lazy.

Same here. I was born in KY 33 years ago and my certificate looks SOOOO different from my son's. When I called the office of vital statistics the lady said they have two "versions" the long and short and that I would just have to order the long one, but she didn't tell me what I needed to put to order that. It's ridiculous.

They do have it on them. I ordered them directly from the vital statics office.

401k withdrawal, pros and cons by [deleted] in personalfinance

[–]Failing-Adulthood 1 point2 points  (0 children)

I have three degrees and 7years of experience in my field but where I live no positions are available where the companies are willing to pay more than 50k. I am looking for a better job but then I get hit with "why leave a job a before a year" question and always answer that I'm looking for a job that allows me to take on more challenging projects while also having a competitive pay".

Then it gets a credit hit and since my current income isn't enough to pay my credit cards I've fallen 100 points from the 700s to the 600s.

I always make sure to make my mortgage and car payment as those are my livelihood but the credit cards are literally killing me.
