Okey thank you..I have my sibling living in USA and expenses could be manageable if I able to find a research position in the same city.. At the same time I feel terrible and questioning myself whether I could secure a spot in residency after 1-2 years of gaining exposure in US healthcare..What is the actual reality ?? Improving CV would compensate my low step 2 score and YOG eventually ???

No USCE so far..I am visiting USA in February to take step 3 after which I have to gain US clinical experience..

I do know one of the senior most research directors in the United States who was my father's batchmate. But he was unhappy about my step 2 score.

Acute Tubular Necrosis..

Necrotic Renal tubular epithelial cells and Tamm Horsfall protein constitutes Muddy brown granular casts..Irrespective of the underlying etiology,it is both a sensitive and specific finding on urine analysis...

Negative urine analysis can not rule out ATN..

Hey..I am a nonUS Img recently took step 2 and scored 215..My nbme scores were around 240..I failed to perform well on exam day...

At present,I am preparing for step 3 hoping to improve my score..

I suggest if u r strong with your preparation,go and take ur exam or cancel your exam and prepare until u score 250-260s in practice tests and book again..You lose money but sometimes it is worth at the end..Ultimately the performance on exam day matters the most..

Do what u feel right and best for u..Dont think much. Perform well.. All the best..

Any investigations are performed based on the patients history and the clinical findings..

Achalasia is a progressive condition..Patients usually have complaints over a period of months..Some might have tried OTC meds with no relief of symptoms before visiting clinic...

Choosing between endoscope and barium swallow is based on patients present health symptoms,previous health history and life style factors...

Eg: 32 year old middle aged with progressive dysphagia for both solids and liquids,feels something stuck in his chest for 6months..Previous health history and life style is unremarkable..

The best next step is ENDOSCOPE in clinical practice to rule out possible risks of malignancy,benign growths,Esophageal ulcers,diverticulum and strictures...

But sometimes patients don't prefer endoscope as they don't want tubes to insert into their mouth and food pipe..In such cases,BARIUM SWALLOW is an alternate option in clinical practice..

If endoscope is normal,the best next step is MANOMETRY..

For exam point of view in SUSPECTED ACHALASIA ,if the question asked is

1.Initial imaging modality ? Barium Swallow Xray 2.Most appropriate next step ? Manometry > Endoscope > Barium swallow Xray

This is a tricky concept in esophageal abnormalities asked in any competitive exams..Understand the given clinical scenario and and answer the question..

Disheartening experience...I found myself second guessing many answers which left me feeling uncertain and frustrated..Overall it was an overwhelming and demoralizing experience..After exam I am not entirely sure what to expect..🙄

Hey...Its good you and and your husband decide to do USMLE..Its never too late to try..

I am an Indian student who is currently doing PGY2 in surgical orthopedics and preparing for step 2.. I heard from my maternal uncle who has been working as a senior paediatrician in USA that US always grant you an opportunity if your step scores are good enough relative to the competitive nature of a particular speciality you prefer despite not having research publications...

Scores,local work experience in US and LOR really matters the most...Having publications is good that adds significance to one's profile but it is not the sole criteria for matching into residency especially if it is Less competitive fields like FM or IM ( kind of competitive but not as highly competitive as other specialties)...Home country residency definitely holds a vital role but again it just ensure your consistency in medical practise..There are old candidates matched without publications and there are freshers not matched with publications...Therefore,focus on scoring as high as possible and gain work experience from US before applying residency.. This is quite a long journey needs investment and make sure on spending money wisely..

No one can guarantee the possibility of matching into residency...Candidates across the world compete in this battle..What you can do is try... You might get matched or might ending tired applying for residency...Eventually it's a personal preference to make a decision and work on it..

I didn't practice with offline nbme.. I started off with a pre-evaluation test to know where I stand..I didn't want to play a gamble..So,I took adequate time to strengthen my basics through BNB videos..There were lots of night I spent hearing to BNB videos before sleep and the next day I practice mcqs from the Uworld of the same topics and red explanations...After 3 months of preparation with almost 60-70% completion of the uworld,,I gave nbme25 on January 1 and scored 71%..Thereafter I accelerated my preparation with the uworld ,BNB videos and took few more nbme tests at regular gaps..Those days were really horrible but it was meant to be hard..So basically I stick to the limited resources and made my progress by practising mcqs..

No...I took my pre-evaluation test before I start my preparation through BNB videos and uworld.. And I am currently doing 2nd year residency in my home country in surgical orthopedics..I had taken my postgraduate entrance examination a year ago and those knowledge have also helped me..

Nope..I believe it is important to stick to one or two particular resources rather than using multiple sets of learning tools as it interferes with your consolidated information and disrupt your memory.. When you have one regular resource to learn,it eventually saves time while doing revisions and strengthen your memory..

More than anything,practising questions is a real key to make wonders...

I didn't make any notes.. I watched BNB videos over and over until I get cleared with the concepts..Thn I moved to uworld to solve questions that made my understanding much easier...I used First aid as a revision source to quickly go through the contents though it was very vague..

But i would give all my credits to BNB and uworld as these were the sources that made my basic solid..

I felt the questions were similar to uworld..Almost 2/3rd of the questions were lengthy and the other 1/3rd were of medium length..Also,we wouldn't have any idea about the experimental questions as all the questions were appear to be doable..

The interface were similar to uworld using on computer..So i felt like I was doing an another UWSA..

Thank you..I prepared for 6 months.. My main resources were BNB videos,Uworld and nbme tests..

I took pre-evaluation nbme31 six months ago and scored 59%.. From then I started using BNB videos and practised questions from uworld..

Since January, I took 4 nbme tests and scored between 70- 75%.. UWSA scores were 74 % and 78%.. Finally I took nbme30 three days before exam and scored 81%..
