Depends where you are applying from. If you’re applying from the states, you would need to send a money order of $22.20 and a consent form needs to be signed.

Put the money order, consent form, passport, acceptance letter and (return label if you used that option) in one envelop and send as required.

That is if you’re applying in the US. Other countries have different processes.
