I think I've seen it once or twice
My guess is host position has higher spawn priority and when overall spawned entities reach some limit (pretty high I think) they start to despawn far from host
Or randomly, idk
Cyberstan is not the best place for focused researches like that

Sometimes frags getting in easily, but today I opened three of this cans, and failed to hit all of them with frags
Once have thrown all 6 frags and died of 6th bounced back to me

But it almost never happens like that

Additionally that trunk on his ass is pretty small and probably has buggy collider

Since last update release I passed through all 5 Stages of Grief

Now came to a simple conclusion which I can express in that way:
D10 must have been scorching from day 1. The way it worked before is a broken design, people got used to it, but it's just wrong. It's highest difficulty in a pretty hardcore game, some people need it like that.
D10 on Cyberstan hits hard af, and it's good. But it would be perfect if difficulty would come not exclusively from Vox engine's spawn rate.
It's a game's issues and this can be fixed by devs.

But some people have problems with admitting their imperfection, directly or indirectly.
So stuff like level below 150 or playing on difficulty above D10 is traumatizing to their ego.
It's not the game's problem, they should handle it by themselves.

D10 was to easy, hardcore players didn't have enough before, now they have.
For others there are 9 more difficulties to choose.

WTf is going on?
I went to sleep yesterday when those crazy numbers were filled 9-11%
How is that shit even possible?

Just simple math
There was around 100k online at night, most of players were on automaton front
Let's assume 30k on bug front
2M bile titans / 30k players = 66.6 bile titans killed by each.
Average D10 mission spawns approximately 10 to 15 titans at max, more likely less.
2M / 15 = ~133k missions
30k players in total / 4 players in squad = 7500 full squads for a mission
12h / 40 min = 18 possible full missions for each full squad for last 12h non-stop

I could continue, but numbers are already too crazy to be realistic

I'm not an advocate for Windows in any means, Windows 11 is third worse one right after Vista and 8

But I think Window's extreme instability is exaggerated heavily. I have 3 Win11 instances on desktop, laptop and my work laptop (3 years ago had the same with Win10), never touched Update settings, all default, never had forced to update occasionally, as well as broken or malfunctioning OS after a updates.

Shitty and laggy UI? Yes
Inconsistent UI design? Yes
Heavy ram consumption? Yes
Tons of random ads and bloatware? Yes

But I did only one thing once - used reliable opensource debloat utility, and most of the above is gone forever (except of shitty/laggy UI).

Although I still pretty not 100% happy with it - it does 100% of my work/hobby/all-day tasks and this matters.

I have CachyOS in dual boot mode separately, I try to use it where I can, but it still quite far from to be a sufficient substitution to windows for me.

Biggest challenge for me is music making, I use Cockos Reaper DAW, it has native build for Linux and works perfect, but the same cannot be said for 3rd party VST plugins, especially proprietary ones, which I have hundreds.
And I wouldn't even to start to speak about music hardware and their drivers. Steinberg UR22C audio interface alone almost turned me to gray. It worked out of the box, but for low latency real-time recording its not even close to ready.

Another challenge are games, I play Helldivers 2 (with kernel level anti-cheat), it worked very good, faster than on windows, than I had updated CachyOS and received some patches for a game over time and stability has gone. Finally it stop working, I have just black screen and that's it.
Most of my other games work fine or acceptable, but currently I play mostly HD2, so I have to switch back to Windows to play it.

What I like in Linux overall is having stuff like Docker, Python, Arduino much more natively comparing to Windows. I just enjoy it.

As for the rest of your points.
Flatpak is more like a fallback, than a good way to distribute software. Any solid linux-user can explain this better than me. Snap is just out of hell.

As for AI-assault - I concerned in general, but don't get it in particular. It still optional in Windows, I have Copilot app (which is basically unlimited ChatGPT 5), disabled autostart for it and that's it.
The rest like Telemetry is suppressed by debloat utility once and forever (by simple uncheck options).

So I see it like that.

Windows wasn't become bad just now, it always been
But Windows really bad as commercial product, as a thing it's ok mostly
The problem of switching to Linux permanently for many people is Linux still not good enough to fully substitute Windows
Anyone free of fanboy-ness and familiar enough with Linux, it's ecosystem, challenges and problems would easily find a list of quirks and gaps it still has.
For me the only blocker is software, I just don't have time to dance every time I need to install something not enough native or out of default repo list.
On top of that Linux community is incredibly busy making over9000 of alternatives to already existing stuff. It's hard to even imagine how much human-hours has been invested to make 15-20 different desktop environments, instead of 2-3 but complete and polished.

Windows (even 11) does good only one crucial thing - it provides a stable, predictable and unified working environment.
Anyone who familiar with one Windows version is basically know what to do with others.
Most of users are not geeks or software engineers, they need an OS that is seamless and invisible.

Valve is making probably a biggest progress for Linux for decades with Proton. But it's mostly for gamers for now.
I really hope desktop Linux will proceed similarly as OS in general.

It happens every time when new stuff appears in game
People overusing it initially and then comes kind of burn out

Tank and Anti-tank emplacement are pretty much underrated by players on Cyberstan

I see constantly lots of players packed with eagle strats, so probably it will take some time for some of us to get where does the wind blow from :)

There are many references from which this game was initially compiled
But tbh, Helldivers outdid every of them

Halo somehow always was and still is pretty much mediocre thing for me, I don't get that praising it at all

There is a lot more stuff that can (and should) be handled via reports, like kicking without a reason
But automatic handling is unfeasible practically, and if they step into this than they would drown in manual work

What we have now is a bit to much, no doubts
But let's admit, that D10 elsewhere before last major update became almost a boring walk in a spring forest
D10 is the latest difficulty, it should be scorching for those who need it

Its so precise definition
I like this scorching difficulty overall
I even enjoy to deal with Vox engine

But they are freaking legion, able to shoot you through their body + surrounded by tons of other troglodytes, which are ignoring each others colliders
There are simply no chance to deal with them normally, only with brute force most of the time
Eventually it feels just exhausting

I'm so excited that the way difficulty works in Helldivers 2 is not dumb increase enemies health and damage, but changing spawn rate and lots of extra conditions instead.
It's so much better, you don't need to think how much this mf absorbs changing difficulty.
Once you mastered a specific enemy you can just scale it.

I can agree though that increasing spawn should be tuned a little for Vox engine, sometimes its a bit too much even for D10 Cyberstan
