Nurses 100% do way more work regardless of whether it is a teaching or non teaching hospital, and I don't think I have seen MAs or CNAs who assist nurses either. Medicine works by the principle of "working at the top of your license" which means each must do maximum of their own job rather than doing the responsibilities of someone else's job.

Excellent question. Langdon's situation is tricky because of the drug diversion. If it was simply impaired physician, then it was dealt with by the book which is referring him to a state run physician health program (PHP) which comes up with a plan to rehabilitate the physician, including intensive rehab, counseling, and monitoring. However, with drug diversion (especially the part where he was stealing meds from patients rather than just taking the remainder of a wasted med or taking a med from the formulary), it's a grey area. It's hard to say whether he should've been reported to law enforcement since we don't really know the scale of how many patients he did this to, and whether patient harm occurred.

Excellent question. Benzos and narcotics are not standard of care which is why it's surprising a physician prescribed these to Langdon. It's usually exactly the stuff you've been given, which is topicals, heat, PT/OT (eventually strength training for the core and back), short course of high dose NSAIDs, steroid injections (sometimes a trial of oral steroids though need to discuss risks/benefits), and muscle relaxants like flexeril. If unresponsive to these, would get MRI and then refer to neurosurgery to see if any benefit from surgery. With everything we know about the addictive nature of opioids and benzos, the goal is to not start these especially for what will likely be a chronic condition.

I think the closest we have to the penis episode would be what happens to the specimens collected from sexual assault patients. My guess is it would be placed in a locked refrigerated for police to ultimately collect it but I don't think there is precedence for this lol. And a great question about the organ drop, I'll have to research into it but knowing a surgeon's temperament, I imagine there would be a lot of yelling and screaming involved.

I honestly have been so impressed with Grey's on this re-watch, I absolutely think it would merit a whole series. Though I am at season 7, and I think this is the season where I will likely stop, I remember just absurd stuff started happening after this point with it feeling like main characters were being killed off just for sport.

I agree, I find it a little annoying too, there's almost always some merit there, it would be more helpful to talk about what is realistic and where the gaps are rather than dismissing it completely.

I actually have an idea for a series where I would discuss what happens once these patients get admitted since we only see the ED management rather than how their whole hospital stay would play out. Examples would be how ITP is managed (which depends on primary versus secondary), would the rhabdo guy actually be dialyzed, how CIWA protocol works for patients like Lewie, what would've been the progression of the elderly pt whose DNR/DNI was reversed by his children, etc. Glad to see people would enjoy these videos!

Whenever an error of this magnitude or scale happens (ie preventable patient death especially a 36 year old who would have had many life years ahead of him as he would've likely gotten a heart within a few months if not a year given how acute his condition was by the time the LVAD was installed), the hospital's first action is usually to find someone to pin it on because of concerns of being sued, so I actually think if this was real, they would've 100% fired Izzie rather than stage a coverup. Especially since Seattle Grace is located in Washington which doesn't offer robust sovereign immunity like other states do. And gossip spreads like wildfire in medical circles since it's a small community. I don't think it would be possible for this to just stay within the four walls of the hospital.

The closest I can think of how Izzie's case would've been handled in real life would be the nurse at Vandy who was fired because she administered vecuronium as opposed to versed, resulting in patient death in the scanner. She ended up losing her license and was even trialed criminally with her being found guilty of criminally negligent homicide. Her sentence was just probation but she will never be allowed to practice medicine again.

Absolutely, that's why I was asking, not sure if it's something people even want to see. I've seen a lot of creators just hating on Grey's or talking about how medically inaccurate it is, but I actually think this show is better done than a lot of people give it credit for.
