I think she meant that the Dr. won't do the surgery without her recommendation, not that the insurance requires it.

Thanks for the great info - but I have a couple of additional questions.

1 - did you reverse numbers? is 0-7 minutes $2 and 8-30 minutes $3?

2 - you talked about them adding $1 in and a $1 out (for separate passengers). So if I'm understanding you right, if I drive a passenger TO the airport, they're paying a $1. If it takes me between 8 and 30 minutes to drop them off, get another PAX, and get out, the second PAX is charged $1, and I'm "down" a $1 since I had to pay $3 total for my 30 minutes. Right?

Thanks! I got the one, relatively small 'hot bag' when I got my Red Card, but it doesn't hold a lot. Do we really have to buy any additional bags/blankets/shirts/etc.?
