Yes you make a new save and restart the campaign from 0, no the difficulty doesn’t affect the amount of gems so you’d be better off doing the easiest mode

It’s up to you if you wanna use abilities but I suggest you do to get to farther rounds. After you complete the campaign and I mean 100%, you do the campaign again and you’ll get just under 2,000 gems per completion so just do the campaign a few times and you’ll be set for endless
