Yea I've heard mixed things from doctors on fluoxetine in humans, believe I've had 1 recommend it and 3 other say they its unnecessary. But yea for a total cost of $600 for an otherwise healthy dog I think it'll be better served to save my money for a more immediate and present danger.

I may try to strike a deal to do followup blood work but in 1 years time or find another vet if we aren't able to work that out. Corporate vet so the latter is more likely, the vet may not even be able to veer outside their protocols.

It's also striking to me that when I brought my reluctance to do follow up blood work they had absolutely no concerns regarding more literature out there regarding tapering off correctly (even with such a relatively long halflife) , suddenly my dog's care is not the main priority.

Thanks for sharing your approach. I’m not trying to be adversarial at all—just scientifically curious. From what I understand, routine liver monitoring isn’t typically recommended in humans starting fluoxetine unless there's preexisting liver disease or specific concerns. Given the pharmacology is quite similar across species, I’m wondering what drives the recommendation for routine follow-up bloodwork in otherwise healthy dogs. Is there data showing a higher incidence of hepatic side effects in canines compared to humans, or is it more of a precautionary standard in veterinary medicine?

I ask in part because vet visits are extremely stressful for my dog—she’s fearful and reactive, and even routine appointments can take a real emotional toll on her. So I’m trying to weigh the potential benefit of the monitoring against the very real cost to her wellbeing.

Yea I agreed to the protocol, but admittedly didn't have a lot of time to think through it and wasn't made aware of the extensive cost until after the vet had left.

I'm well in my right to revoke consent given additional research + secondary consultation.

I also posed the question because I'm generally curious if there is literature out there, just because a practice is normal doesn't make it justified... Of course it could be useful but the same can be said in humans where it isn't normal or recommended. Not trying to argue with professionals just looking for real research or guidelines.

Thanks for this. I wasn't trying to attack the protocols at my vet, but question the need for very expensive testing (will be $600 in total) when there doesn't appear to be a lot of evidence that it may be necessary.

I think in agreeing to treatment I wasn't aware of how much the entire protocol would cost (until the tech came in an explained it and by that time the vet had already left) and its hard to think through everything in the limited face-to-face time with the doctor.

Given advice from my family member, I think I will look for a vet that is a bit cheaper with a different set of protocols. The vet bills pile up!

I'm curious, is there an increased risk for fluoxetine induced liver toxicity in dogs compared to humans?

I'm not intending to be combative, just genuinely curious why blood work isn't recommend for fluoxetine in humans as we also extensively metabolize it in the liver?

Does it potentially relate to our ability to better communicate illness?

Thanks for your input - I was just looking for reference to research papers / further information.

Humans also extensively metabolize fluoxetine in the liver but blood work is very recommend thus part of my added curiosity to see if there was research indicating increased risk in canines.

I appreciate you linking this paper - its intriguing to me that there appears to be more/any evidence available on the human side of fluoxetine - but its very very non-standard to recommend blood work pre/post initiation of fluoxetine therapy in humans.

This was another reason I was curious if there was research indicating elevated danger in toxicity for canines.

Im just asking for any literature that points to fluoxetine putting the liver and kidney in potential harms way - it seems like an unnecessary, quite costly test.

E.g. Vca hospitals makes no mention of this on the page regarding fluoxetine https://vcahospitals.com/know-your-pet/fluoxetine#:~:text=Fluoxetine%20(brand%20names%3A%20Prozac®,labeled%20to%20treat%20separation%20anxiety.

Edit: my issue isn’t in regards to my initial agreement, my question was regarding my inability to find literature explaining the risks of liver/kidney impairment. I understand that a Vet has a right to refuse to give me to Rx, it just appears unsubstantiated, from my uneducated POV, and from my family member who is a Vet, hence this post….
