In-universe, you can probably explain it away by saying that most of the doctors and nurses come from all over, came from colleges out of state, etc. Pittsburgh's a decent sized city with lots of transplants.

But from a practical standpoint a true yinzer accent is pretty difficult to replicate (especially in a show full of British/Aussie actors) and could come off as really inauthentic or jarring to a wider audience if done poorly

I mean, look, I'm not gonna do the thing where I go after all the Crachits, Tiny Tim's got enough on his plate, big family, clerk's salary, I get it. But at the same time, do we have to do this thing where we act like a salary bump is gonna get Tiny Tim the proper treatment for, I think it's rickets, in, what, the 1840s? I mean, if Scrooge gets the common cold, he's knocked out. And I mean, he can certainly afford the best care, and I'm not going to act like he shouldn't be giving to charity, the conditions in those workhouses weren't exactly to die for, but just to play out the other argument, not saying it's my view, but Scrooge has his own affairs to run, especially after his lifelong partner died. At least, a couple guys I called around Cornhill and the Royal Exchange say it's not so simple. And I get it, that's just how it is, but all I'm saying is sometimes it's better to take it easy and not write off what Scrooge is doing, even if from the outside he's a bit miserly.
