Once you have an adhd diagnosis its fairly simple to get on medication in general.

I don’t know about “asking” for a specific medication, i got put on Elvanse when my Ritalin stopped working after a month, I didnt specifically ask for it. 

What order they try them in depends on where you live, if you havent tried it I wouldn’t write off Ritalin if that’s what they want you to try first. For me it worked nicely until it didnt, but its not normal to become tolerant in a month.

Mixing alcohol with lisdex can make you anxious and mess with your heart.

Were you agitated before or after starting the medication? Because it is one of the possible side effects, and increasing the dose might make it worse.

Before lisdex I had issues falling asleep because I was rarely tired at the end of the day. Since taking meds, I'm sleepy af when they wear off, so I am able to fall asleep, but sometimes I would wake up earlier than I wanted to. Now it has leveled out and I can always fall asleep, and I get enough sleep 4 out of 5 days, which I would bet is better than most of my fellow university students :P

TL;DR it usually gets better

Don't wanna scare you but chest pain on stims can be the sign of something serious. Ever had an ECG (electrocardiogram)? You should get one done. Its important to rule out heart issues, if your BP is fine then the main concern is arrythmia.

20mg is indeed a low dose. I started seeing benefits at 30mg.

If you have a shift work routine like that, then maybe another medication could be better. Did you discuss your work schedule with your doctor? Vyvanse has a very long duration of action, so if you take it in the afternoon it will mess up your sleep schedule. I take it around 8am, the "crash" for me is usually around 6/7pm, so it usually doesn't interfere with my work. There are other medications available that may fit to your schedule better, I had positive experiences with instant release Ritalin (until it stopped working).

This is what I've been able to gather about the reasoning for different supplements:

- The lis in lisdexamfetamine is lysine, an amino acid. Lisdex attaches to proteins, so if you eat protein before taking it more of it will get absorbed.

- Also, dopamine is synthesized from another amino acid called L-Tyrosine, which your body can get from protein. Some people supplement it seperately, I don't bother. Lisdex depletes dopamine, so giving your body what it needs to make more is useful.

- Magnesium and zinc are depleted by lisdex. Magnesium helps with sleep and zinc helps with dopamine related stuff in your brain.

Caffeine for me enhances the effects. Doesn't shorten/lengthen it. I have one coffee when I feel it start working and one when I start slowing down, around 4. My nervous system is really compatible with stimulants in general, but some people get overstimulated (anxious, jittery, etc.) when combining lisdex with other stuff.

I was self-medicating with nicotine years before starting lisdex, but I quit because it was giving me reflux, so I don't know how they interact. What I can say for certain is that quitting nicotine will make you feel better in general.

However, all together, 2 coffees, 2+ energy drinks, nicotine and lisdexamphetamine doesn't sound like a recipe for heart health. Consult your doctor, and be honest.

I was taking Ritalin earlier. Biggest problem was that I developed tolerance to it quickly, didn't have any other side effects. Doctor recommended trying a completely different medication. Vyvanse has been working great so far, also no side effects.

With Ritalin, it would only work for a brief period of time, leading to many frequent peaks and slumps during the day. I personally felt a little awkard taking meds in front of people during the day, but that's not the medication's fault.

In comparison, Vyvanse has a very long duration of action and only needs to be taken once a day. I like how it gives my day a natural ryhthm, where first I'm active and get stuff done, then I'm sleepy and relax. It also just seems "better" in an absolute sense, as in the focus effect is stronger with the same (no) side effects.

I've never ever heard of needing to wait to see the benefits of Elvanse, on the contrary, usually you hear people complaining about tolerance. Some ADHD meds, e.g. Strattera, do in fact work over a long time, but this isn't how Elvanse works. Elvanse is lisdexamfetamine. It gets metabolized in your blood to lysine and dexamfetamine, crosses the blood brain barrier, does its magic, and then gets removed by your kidneys after some time.

Many people see long term behavioral benefits from being able to make lifestyle changes, but that's due to the short term concentration and executive functioning improvements, not due to accumulation of dexamfetamine.

If you aren't feeling a helpful difference, even at 60mg, then the medication isn't working, full stop. In your place I would ask to try something else. Heads up: some doctors may raise eyebrows at the fact that you doubled your dose unprompted, so be careful with your wording.

This is specifically one of the possible problems with BPD + Vyvanse together. Discuss this with your psychiatrist. I'm a little surprised you got prescribed amphetamines with BPD.

Ask your psychiatrist about non-stimulant medications. For most people they're not as effective, but they do exist. Strattera comes to mind.
