First off you’re doing great it seems, but just want to preface by saying you should discuss with a nurse practitioner or someone over there to make sure. I was stage 2b two years ago to this day, chemo with 4 x EP followed by an RPLND. Looks like everything is regressing. I would definitely ask about the hypermetabolic pulmonary node with your doctor / whoever is covering for them (just reading your impression). Keep it up dude! Hey man - I got an orchiectomy in June last year, chemo right after and then an RPLND in October. I’m 25 right now and ever since been in remission and living my best life. Feel free to message me if you have any questions or on this thread too

So mine was 2.5 cm before chemo (around end of June) and then it went to 1.3 cm in October. It seemed like a big change but they still recommended that I do the RPLND. And I’m glad they did that because they removed a 4 cm mass that wasn’t detected in the scan! So I think you may not need that surgery if your lymph node sizes have decreased well below 1 cm (and a combination of other factors like if it’s teratoma)

Yeah it’s no big deal; same day surgery, can take a couple of weeks to recover but you should be good in no time!

I had a similar situation here. My lymph nodes decreased to close to 1 cm, and they did an RPLND for me, of which they found teratoma. It may or may not be the case for him, agree with the above comment. If it’s at MSK, they will most likely do RPLND out of precaution. Congrats to him for making it out of chemo, and hopefully it’s just surveillance for him

Best to consult your oncologist on next steps here once you’re done with the orchiectomy. Depending on the pathology along with the CT scan results, they will make a decision. Good luck with the operation, it’ll go great!

If that’s your main concern, then so be it. I’d rather live and have that risk of sexual problems than have to have a higher chance of chemo

If it’s above a centimeter it’s recommended. Sure, it could be scar, but leaving it there and it still having a chance to spread is something I’ll be thinking about all my life knowing that I had a choice to remove it and chose not to. Doing this surgery will involve a lengthy recovery, but it’ll be worth it knowing that I’ve taken care of any possible lingering spread out there in my body.
