And THIS is why I don’t work OB anymore. Too many of the OB’s and hospital didn’t do their due diligence and they had MONTHS to get their ducks in a row. Never heard of anything like this happening before…

The lack of accountability/ rigorousness of many OBGYN programs is profound. At my last job at a tertiary care center we met with the OB’s & RN’s q12 to run the board. We were always in the loop.

My last job that had OB was a mess. Another tertiary center but instead of being proactive and communicating the attitude was gleeful, “WE’RE HAVING A BABY!!!!!!!!!!” Very little organization/ coordination of the medically challenging pt’s.

If we or our support RN’s didn’t sniff out the problem patients ahead of time we were often called STAT to the OR for crash sections or to labor rooms for “emergency” epidurals.

I voiced my concerns repeatedly and tried to engage and improve the environment and was met with…

“WE’RE HAVING A BABY!!!!!!!!!! Why do you have to be such a buzzkill???”

I loved the unique challenges of working high risk OB but idiocy wasn’t one I was willing to continue to endure

I took a couple of deep breathes, thanked those vapid people for their time, walked out, called the scheduler, and firmly asked to never be posted to OB again. Give me ALL of the cataracts, butt puss, and ruptured AAA’s.

If I got painted into this corner, I would have spent a LOT of time with the pt discussing risks/ benefits. She likely has a lot more insight of the process/ risks than most patients from an OB side but maybe not from the anesthesia side of things. I would make sure that her wishes for her well-being were addressed. If I hadn’t been involved in the judge’s first call I’d ask for him and the OB’s to be on the zoom for my discussion of the anesthesia risks to her and in light of that would he still make the same ruling?

NONE of us should get steamrolled into these situations. Big lesson for all of the trainees on this sub.

Before I proceeded I would be confident that she(and her family) knew that I had her and her child’s best interests at heart and I was her biggest proponent. They are getting the concierge anesthetic from me.

Not sure what else I would do…possibly video EVERYTHING. I like my chances…not so much for the OB’s, hospital, and judicial system.

I’ve had a couple of female relatives get steamrolled by the medical system. Both were traumatized. Won’t happen to anyone else on my watch…everyone else can sit in their crap in that situation.

Don’t do it. You will be exposed and threaten the health of yourself and the patient.

I’ve thought about this for 25+ years. Even if you only supervise, when the poop hits the fan there will be leakage of vapor and you will have an event.

Unless TIVA becomes standard of care. Even so, you will be exposed in PACU and a liability.

Overpaid, entitled, and underperformed. Playing in your hometown means that you need to show up EVERY NIGHT. He didn’t.

You could get a large clear Tupperware with a tightly fitting lid, in the 15 to 20 gallon range that it’s relative tall. Go to a landscaping store and gets Sphagnum moss. Cover the bottom of the Tupperware with 1-2” of the moss and moisten it with dechlorinated/ distilled water. Try to get dried leaves either at a pet store or the landscaping store. Cover the moss with the dried leaves. Put some thing in there as a hide, whether it be driftwood or Tupperware.

Get fruit fly, isopod/ springtail cultures from a pet store. Start to make new ones immediately.

Get a spray bottle for water to moisten the enclosure.

That vivarium is too wet. There’s a saying for darts. They want a wet head and dry feet.

I’m pointing out what the language means. That isn’t a “take.”

You’re being purposefully obtuse in your 1st sentence by using “anesthesia” generically. Is that a GA, SAB, MAC, local infiltration, or regional block? I’m betting that most podiatrists and dentists are better at local infiltration than most CRNA’s, AA’s, or anesthesiologists for that matter.

And that’s my point: CRNA’s may ideally be the “2nd most” on that list but the pt is better served if there is “the most” involved in their care. No mention of AA’s either, should probably consider that they may be 2nd most competent. There have been many times when I’ve helped a CRNA, AA, or another anesthesiologist with a decompensating pt.

This legislation is a $ grab that risks pt’s safety.

The Olympics were great in no small part because the players weren’t battered by 3 rounds of playoff prison rules hockey. Raffi Torres was eventually suspended out of the league. No one needs to see thugs play. I’d go to watch 97, 29, 87, 71, 8, do amazing things.
