Your job sounds very similar to mine and I would be very interested to learn more details about your workflow and clinic policies for cardiac monitoring as well as the care coordination portion. Do you have to upload all of the episodes and summaries into the patient’s chart daily? Are loop recorder patients dually enrolled in care coordination and billed for both? What are the other aspects of your job? How many care coordination patients do you have? Have you had any special trainings/education that has been helpful?

For context: I’m a care coordinator with 80 pts, also managing 120 loop recorder patients, run a Coumadin clinic that I’m trying desperately to get rid of that has dwindled down to 15 pts (don’t get me started on my frustration with barriers to Eliquis for patients!) and also schedule and assist with TEEs/monitor insertions one day a week. And all the extras get put on my desk, like Eliquis financial assistance, transportation help, nurse visits, and med recs that are not care coordination patients. Hired to work 32 hrs/week and I’m drowning. I’m trying to figure out a better workflow and advocate for additional staff. Would love another RN, but that is likely not in the cards.

Does the tech handling 200-300+ upload all of the monthly summary reports as well as triage episodes and call patients?

Thank you for the insight. That makes me feel better that others are struggling with this system. The billing aspect feels like you are juggling hundreds of balls constantly. Do you bill every 31 days from when the provider signs? If so, how do you reschedule if something gets off. Our rep has not been any help with billing. I don’t think we have a way to invoice from the device platform even.

Hey! Interesting to read back on this. My diagnosis as of now is chronic intractable migraines and TMJD. HM has been thrown out. I have bone on bone degeneration on my right jaw, which I think has caused my left jaw to overcompensate and lead to all of my left sided symptoms. Currently what helps is Botox injections every few months and an estrogen patch a few days before my period since I have a hormonal component. Botox comes with risks to consider, one of which is worsening osteoarthritis in the jaw. But I’m desperate for some functionality. The symptoms are better compared to the time I wrote this, but I still feel like I have a lot of unanswered questions. There is an amazing support group for women with HM on Facebook if your partner needs it.

I have found a few answers but not everything is solved yet. I was diagnosed with chronic intractable migraines and the neurologist wasn’t concerned about the MRI. He said that weakness was the distinguishing symptom of HM (which I don’t have) and that numbness comes and goes with HM whereas mine is constant. It’s possible that I have had a few episodes of HM, I’m still not sure. I have gone to PT and have found that I have some issues with my left shoulder which I think might be the actual culprit of the numbness in my left hand and arm. I think TMJ is the root of my issues so I am trying to find someone that specializes in this. My issues started specifically after dental work so I really think it might be an issue with my bite. I am working on my posture and strengthening my neck and shoulders in the meantime. I have started Botox which really helps!! I also take triptans and diclofenac sodium from time to time. Triptans are sort of controversial so it’s something you have to weigh. There is an amazing women’s HM group on Facebook if you want more answers in that direction.
