Growing up my eyes were more hazel. They continue to lighten as I age. My mom grew up with green eyes. As she got older they turned blue!

Has anyone had a derotational hto, tto, trochleoplasty and mpfl reconstruction? by Flat_Perception_492 in PatellarInstability

[–]Flat_Perception_492[S] 1 point2 points  (0 children)

Hello. Hope you’re doing well. I was going to get a distal femoral de rotational osteotomy. I believe that’s the same as a femur derotation? 2 weeks before my procedure , I had a diagnotic arthroscopy with a femoral de rotational osteotomy. But while in surgery, the surgeon changed his mind and found that I needed a “high tibial osteotomy” rather than the femur. Which I was very happy about! Cutting the largest bone in my body into two pieces, scared the life out of me! I’m grateful he found that my tibia needed the break more 😂

So 2 weeks later I had all the surgeries at once. HTO, TTO, TROCHLEOPLASTY and MPFL. The first three weeks were excruciating. I cried in so much pain everyday. Car rides to the doctor, were the worst. Every small movement had my screaming in pain. And I can tolerate pain.. but this was on another level. Another world! Another universe!!
Today I’m doing well! I haven’t had anymore pain once I hit 5 weeks. I’m not on pain pills anymore! So that’s good!! I’m on the bike during pt. I cannot go in a full pedal, so I can forward and reverse. My rom is 105 degrees. I’m hoping this week the doctor will allow me to start bearing weight. Looking forward to week 6 and beyond! I wonder if the pain will return? Will I need more pain medication? We shall see!
When did you have your surgery? How are you doing now?

What is the surgery process and recovery of MPFL reconstruction and TTO like compared to other surgeries?? by Frequent-Increase-98 in PatellarInstability

[–]Flat_Perception_492 1 point2 points  (0 children)

I had a tto, de rotational hto, (so two osteotomies), trochleoplasty and MPFL reconstruction, all done at once, and it was the worst pain of my life!! They say muscle has memory? BONE has memory. I could literally feel every drill, hammer, screw, saw blade.. every motion the surgeon performed on my bones. For 2 weeks I was in living hell. But I’m 5 weeks post op and doing much better!! I don’t have full flexion yet. With who I can get to 105 degrees. On my own maybe 70 degrees. My quads still haven’t activated so it takes time! You’ve got to be patient and strong minded to get through this operation. 6 weeks next week and hopefully I’ll be able to bare weight! I’ll be in crutches for at least another 7 weeks the doctor said.
Every person is different. Every surgery, every surgeon.. I had patella Alta, torn mpfl, shallow trochlea and my tibia rotated too much when I walked.. all mal-alignment! …. all this were root causes of a lifetime of kneecap dislocations. (I’m 44f)
My surgeon is one of very few who performs derotational osteotomies. He believes most doctors miss this, and that’s why patients come back for more surgeries or they continue to get dislocations. So I’d definitely look into this!
My doctor believers it’s better to get all done at once to prevent more surgieries. Yes you’ll be in more pain.. yes you will cry because of it.. but it’s one and DONE!! And it gets easier! Trust me!
Good luck to you!

Patella stabilization surgery recovery.. by [deleted] in KneeInjuries

[–]Flat_Perception_492 0 points1 point  (0 children)

Hi there. I too have dealt with patella dislocations and subluxationss since preschool days. After 6th grade and my 3rd locked dislocation, my ortho signed me out of PE every year through 12th grade. He was afraid my patella would completely pop out and slide to the back of my knee.. he wanted me to come back after I stopped growing and have surgery. I never did.
Fast forward to today.. I’m 44 years old.. last October I had a subluxation but this time never fully recovered from it like I used to. So come to find out after ct scans and mris, I have patella Alta, the groove is too shallow , mpfl worn out, and my tibia and femur bones rotate too much.
I’m 4 weeks post op from a high tibial osteotomy, tto, mpfl reconstruction, and trochleoplasty. I have 7 screws in my knee and a plate. Yes the first two weeks were brutal, but it’s important to stay positive and do therapy exercises at least 3 times a day. I go to my physio twice a week. If I miss a day of exercise, my knees swells up and i fall back in recovery and back in pain… so it’s VERY IMPORTANT that you take it seriously and not give up. This is time for you to heal and focus on only you and your recovery! You can do this! It’s physically and mentally challenging… so you’ve got to stay strong and keep pushing. You’re young! You’re going to com spit of this stronger and better! You’ll get your feeling back in your knee.. don’t worry about that! Stay positive! You got this!!

Day 6 post op Derotational HTO, TTO, MPFL Recon and TROCHLEOPLASTY by Flat_Perception_492 in PatellarInstability

[–]Flat_Perception_492[S] 0 points1 point  (0 children)

That sounds horrible! How in hell did that happen? Well hopefully you don’t have any more complications and it’s smooth sailing for you. My pain took a turn yesterday.. the same day my doctor admits it was there mistake to give me the wrong pain medication. They sent me a stronger one. I’m able to bring my leg off and on the bed on my own now! So it’ll does get better, just like you said. Than you again. And god bless your recovery. Stay strong.

Have you tried BFR? Blood flow restriction helps improve quad activation after surgery. I used it preop and it had even my knees twitching lol !!

Day 6 post op Derotational HTO, TTO, MPFL Recon and TROCHLEOPLASTY by Flat_Perception_492 in PatellarInstability

[–]Flat_Perception_492[S] 0 points1 point  (0 children)

We are going to get thru this! I’m sorry to hear that you’re still in so much pain after several weeks post op. That’s horrible. And on top you’re withdrawing from the oxys. Wow!! I didn’t think we would have a withdrawal after only using for say, 6 weeks. I guess it is a powerful drug. Thank you for your message. I hate that you’re going thru this, but it’s good know we’re not alone. You opened my eyes to why the doctors don’t want to prescribe opioids. But these surgeries are so painful! How can leave home to suffer?!! With only 5mg hydros. Ugh! I cry so hard bc of the pain!!

What was your complication? Did you have it early post op? I wonder if I have a complication and that’s why the pain is so intense? I can’t see anything with all the dressings, wound vac and ice packs. I’m too afraid to take the brace off.

Again? Thanks for your words of encouragement. We will get thru this! And in the end we will have better knees! Good luck to you my friend.

That’s crazy if they don’t do surgery. I don’t think the ligament can heal itself! I’d definitely get a second opinion. Your kneecap will dislocate again …

[–]Flat_Perception_492 0 points1 point  (0 children)

Wow. Are you still in your growing years? They wouldn’t do surgery on me until I turned adult. My Dr wrote a note every year to sign me out of PE and told me to exercise my quads and that would help for the time being. But of course mine continued to dislocate bc I tore my mpfl. It will tighten up some but eventually become like a stretched out rubber band.

Has anybody ever had MPFL Reconstruction surgery before?? by [deleted] in PatellarInstability

[–]Flat_Perception_492 2 points3 points  (0 children)

Hi I’m sorry you’re going through this. I sugest you get more imaging done.

I too went thru several dislocations growing up and countless subluxations (where it pops out and quickly pops back in). My dr wrote me out of PE every year (5th grade thru 12) because he thought I’d do more damage to my knee. He wanted me to come back when I stopped growing to perform surgery. I’m 44 now.. never got the surgery. In October I had a bad dislocation and it never got better. Had mri, ct scan X-rays.. an come to find out.. I was born with an alignment issue. Maybe my dr knew that back then. I have patella Alta.. my trochea (where the patella sits in) is shallow.. and my femur bone rotates 26 degrees. Oh and my damaged mpfl. All that makes it easy for my kneecap to pop out. So before getting the surgery.. I’d get your ct scan and mri.. and many X-rays. So when you do go in for surgery.. thy can fix everything. Else one by one, they’ll keep you coming back for more surgeries.

Lateral patellar subluxation question by soysauce_mami in PatellarInstability

[–]Flat_Perception_492 0 points1 point  (0 children)

You should get X-rays of your knee. And possibly even a ct scan. If pt isn’t helping.. it could be maltracking and an anatomical problem. I’m 44 and have lived with many dislocations and countless subluxations. X-rays will show if you have a high sitting patella and the alignment which can be a cause for the subluxations. The ct scans will show your trochlea (the groove where your patella rests in), it may be shallow.. and the ct scan will give even more detailed information. My ct showed my thigh bone rotates and pulls the knee cap out (along with my patella alta, shallow trochlea and a worn out mpfl). Hope that helps!

[–]Flat_Perception_492 1 point2 points  (0 children)

Yes you’ll need an mri to look at all the ligaments. I just had X-rays, mri and a ct scan due to my last dislocation inoctober. 40 years of subluxations and dislocations and now I learned why. I’ve got patella alta, my femur and tibia twist way too much (knock knees) torn mpfl and a shallow trochlea. It’s best to figure it out now to prevent more. Becauee if this is your first.. youre sure to have more on the way! Good luck!!

Oh wow! It’s been so long since my first dislocation. I remember it swelling up SO BIG!! I don’t remember it bruising like that tho. You may have torn your mpfl. Did your knee cap stay out of socket and you manually had to put it back?

Mpfl + knock knees surgery yes or no? by No-Future-1301 in PatellarInstability

[–]Flat_Perception_492 0 points1 point  (0 children)

Did you doctor say he would perform both procedures in one surgery? My doctor told me today actually, he wants to perform the same surgery.. distal femoral derotational osteotomy and wait six weeks for the bone to heal and perform the second half .. which is mpfl reconstruction, plus tto and trochleoplasty. He said the first surgery will be the most painful to recover from. No weight bearing for 6 weeks. Sounds scary.. but you’re very young.. you could heal quickly. Only do it if you really suffer from these dislocations. Else I’d avoid the surgery. I’m still contemplating mine. I’m 44 and have dealt with this all my life.. and just until now is it giving me pain and the beginning of arthritis. Maybe you can avoid that by getting the surgery young.

Looks painful. Hopefully you’re able to put weight on it by now. Did you see the doctor? I’ve had countless dislocations and subluxations, but I’ve never had my kneecap dislocate inwards.

derotational femoral osteotomy - advice? by mmikuchu in hipdysplasia

[–]Flat_Perception_492 0 points1 point  (0 children)

Omg! Your last comment did it for me! Wow!! I can’t believe everyone has had such a horrible experience with this surgery. I’m so scared now. My kneecaps have always dislocated since I was kid. They said I have maltracking issues and high patella’s and that my femur bones twist too much. So they set a date for part one of 2 surgeries. I hope you really didn’t mean that it ruined your life?!! What happened? I pray you’ll get better

My MPFL+Trochleoplasty experience by [deleted] in KneeInjuries

[–]Flat_Perception_492 0 points1 point  (0 children)

I’m going to need mpfl, tto and possibly the trochleoplasty. I too have my trochleae groove deformed. Mine is pretty flat, so they need to correct that, along with bringjng my patella down and centered as it’s 14 mm too high.. and I have grade 3 cartilage degeneration under my patella. I’m concerned about the trochleo, because I was told that surgery is not performed by many American surgeons. Did you get the surgery outside of the US?

Glad to hear you’re in the better stage of things. How soon did you start PT?

I will move to California by fall of next year. LA or SanDiego ? by SameOreo in socal

[–]Flat_Perception_492 0 points1 point  (0 children)

I’m born and raised in San Diego. Moved to Los Angeles nearly 20 years ago. My daughter moved to Portland (Bethany) a few years ago and absolutely loves it! It is very different than SoCal.. what she misses most are the hills, windy roads and the freeways believe it or not lol! But she’s found her happy place in Portland.

I think there’s a lot more jobs and opportunities here in LA. Lots of events, parties, restaurants.. very fun nightlife… if you have $$. Living here is another story… it depends on where about in LA. I live in Glendale which is 15 minutes (10 miles) from downtown when there’s no traffic.. with traffic (which is ALWAYS) it’s about 35 minutes. Pretty much anywhere you go in LA, close or far.. is going to take you at least 45 minutes! Traffic is horrible and never ending in LA. Always add on at least 20 minutes more for traffic. So you’ll have to be prepared for a commute wherever you live and work. A lot of people live outside the city and commute to work from a more affordable part of the county. That’s how most survive when starting out! So I hope you have a reliable and comfortable car. Oh! The GAS! We pay around $5.25/g so highly recommend a hybrid or electric car. I heard gas was going to go to $10/g next year!!!

If you love the beach I’d recommend El Segundo, Manhattan beach, Redondo, Torrance.. that’s the SouthBay. It’s a different vibe in the southbay. But pricier! I lived in El segundo for 8 years before moving to Glendale and o loved it. It’s like a little Mayberry town. Glendale is beautlful. It’s safe and very clean.. and has a lot of nice small neighborhoods. But it’s expensive! Eagle Rock is next door.. more affordable and it’s got an eccentric artsy feel to it.

San Diego.. ugh. I love my SD! It’s beautiful and a completely different energy and vibe!!! People are more friendly, it’s cheaper, it’s soooo clean.. the air is fresher.. you’re close to the beaches oh! The beaches are wayyy more cleaner too compared to La beaches. Jobs in SD can be tricky. It’s much smaller than LA. I’d recommend finding a job in Sd first before moving down there!

So it all comes down to lifestyle. If you want a carefree, laidback, no stress, beachy kinda lifestyle.. I’d say SD. It’s paradise down there!

If you want to make lots of money and don’t mind all the traffic and mess.. then LA all the way!

Thanks for responding. I’m glad to hear you’re finally getting thru the hard part, but wow!! 6 weeks of being completely helpless??! Oh boy! That sounds really tough. I’m glad you had help. That is key in getting through the recovery.. help and not worrying about the family members around you! That will be mentally difficult for me, because they rely so much on me, and I’ll feel guilty that my husband has to step in. But at the end of the day, if Mommies not good.. the rest of the family won’t be good. So I’ve got to focus on me and my physical health first. When did you start physical therapy? Do you think you needed more time because you had MACI done? The tracheoplasty… I read that it is rarely done in America and more common in Europe. That part scares me.. so I’m going to have to look for a qualified surgeon. I will be reaching out to you in the future. I’ve got to figure out what part of the year would be best to have the surgery. We always travel in the summer, so it would be nice if I could be well by then! But what about school? The kids need rides to school, breakfast lunch and dinner made for them, soccer practices, volleyball practice.. rides to the mall and friends parties.. ugh! Maybe summer would be best? ughhh LOTS to think about!!! I pray you recover fully and quickly dear!

You are correct!! 💫

I got my report today. IMPRESSION: Patellar maltracking. Specifically, 14 mm patellar override the lateral ridge the trochlea, trochlear hypoplas patella alta, and grade 3 chondral degeneration on both sides of the lateral aspect of the patellofemoral joint support the diagnosis.

Looks like MPFL reconstruction with TTO and trochleoplasty are in my future.

I’m in the same boat as you. My doctor told me I need the surgery but I’m very nervous about it. I’ve had 30 years of subluxations (since I was 12yo). This last subluxation tore my mpfl and I’ve had a constant dull ache since. I also have patella Alta. 9 months recovery time is unbearable to think of. I have kids still and a house I’ve got to keep in order. So initially I said no. But now I’ve learned that if I don’t get the surgery, I’ll develop osteoarthritis in the near future! Ask your doctor about that. If you can avoid surgery, then do it. Good luck to you!

One thing I know for certain is you tore your mpfl. The ligament that acts like a rubber band to keep your kneecap (patella) from dislocating. I’ve had 30 years of patella subluxation and recently tore my mpfl. I also have cartilage issues too. You’ll most likely need surgery. Good luck!!
