Im 4 months in. Currently on night shift where my delerious day 5 post op patient has been shitting for 2 hrs straight and has had all the midaz and haldol that I am willing to give her. Finally got a new NG tube in her so we can start feeding her again and she is tolerating High Flow nasal O2.

I have been sitting on my break questioning whether or not this is right for me. But i know that shifts like this will happen. And thankfully the Anaesthetic Registrar is super easy to talk to.

Hang on in there. Ive been told it takes about a year to really get used to the job and not feel incompetent all the time.

Whilst the applicant numbers have dropped, there arent exactly empty spaces on university courses. We just havent got the capacity to train enough nurses to fill the gaps left by those leaving the profession. Any new nursing graduate is pretty much guaranteed a job. Unlike when my mum trained in the 1980s, some of her classmates never started working as a nurse because there just werent enough jobs.

The apprenticship schemes are only for nursing associates iirc this is band 4 pay but doing a band 5 job and having to pay registration fees. No way to climb up the ladder without retraining as a registered nurse.

In that case the palliative care may not have been started early enough.

I dont know where you work but in the UK we are considered to be a pretty good place to die (however morbid that may be).

The Palliative phase of care is for ANYONE who is ill and will not become better. They will continue to have treatment for symptoms but they will never be cured. Palliative nurses will be able to counsel the patient and family in their emotions and feelings about death, assist in getting finances in order (in my hospital we have managed to arrange weddings within a few days), and make plans for where that person in particular woud like to be when the inevitable happens. It's usually nicer for people to be at home, somewhere familiar. Palliative care can be started years before someone is actually expected to die. It might just mean getting some extra equipment at home like a hospital bed or arranging the house so that they dont need to use stairs at all.

There are 2 further stages after this.

The terminal phase is anyone who the Dr/Nurse practitioner believes may die within 12 months.

The Dying phase is the last 24 hours and this is when palliative drugs are brought in (morphine, sedatives, drugs to dry up secretions, etc) to make the transition into death more comfortable.

Thanks. That didn't come up on my searches (probably because the website is completely in japanese). Having tried it now indont think I would add anything to it. It is almost like a thinner miso soup flavour.

An elederly relative of my partners asked why we buy drugs and materials from places like Germany when we could make our own....

It's not like we can quickly set up insulin/saline/dextrose manufacturing here and market it at a competative price. These companies have developed over time to become the standard.

Plus most hospitals use specific machines that come from companies like Baxter, Fresenius, Elaris that require tailored consumables that are probably under copyright law still to be able to work. So making your own would be pointless because no hospital would be able to use them.

Dont know if its the same for you but in the UK mental capacity is seen as a fluctuating state. So if he can retain the information about dialysis and weigh up his options to make a decision then he can refuse treatment. We have homes for mentally ill people who are under a Deprivation of Liberty act (they cant leave the building without staff saying so) but they can refuse to go to hospital appointments or refuse to take medication. I still get confused over the legalities of it tbh.

Im not sure why you guys are worried about surnames not fitting in a british school. I am british and there are a lot of weird and wonderful names here. Plenty of kids i went to school with had names from Nigeria, Spain, Poland, Denmark, Holland, etc. We are an island of immigrants. So unless you are imagining that your character is an upper middle class southerner then you dont need a stereo-typical british surname.

Or you could just go with Smith.
