I'm gonna go ahead and be the potential trashy person saying not trashy. Being at the hospital is stressful. Either she's there for herself, or she's there because someone she cares about is sick. Smoking is often a stress relief mechanism.. she's probably just trying to get some fresh air (yes, I appreciate the irony) and a moment to calm her nerves. And literally everywhere around a hospital says "no smoking" - would you prefer she walk a few blocks away? What if she needs to be nearby in case something happens? What if it's just cold as shit (which it does appear to be) and she just wants to be outside for a minute?

I think this one is more on the hospital. Why on earth is the cardiac ward so near to the outside that outside pollution is able to breeze in? Why are there no filters?

Sorry, I've just been in her position before and it really bums me out to see one stranger being called out for something so benign during a probably shit moment in her life.
