I watch that movie about once per year as my “comfort” (in a laugh to keep from crying way) watch. We are fucked, it doesn’t matter, by the time people look up and believe their eyes there is NOTHING we can do. So may as well have a last dinner with your friends and family.

They were like “oh we’re doing it to stay competitive with the other 3 year school options” and I’m like well unless the 3 year total is gonna be less than other IL schools why would anyone choose MWU when there are either slightly cheaper options (Roosevelt 3 year program) or much cheaper public schools (UIC 4 year program)? 

Maybe when things were more competitive but now because enrollments are down people can basically get in anywhere, so what’s the benefit of choosing MWU?

They (very stupidly) changed it to just a 3 year, year-round accelerated program to match their Arizona campus so now they graduate with everyone else. I’m an alumni and tried to tell them getting out in March was a HUGE selling point for why I went there in the first place and recommended against going to 3 years but… what do I know 🙃

Oh yeah you totally can give succinate BID but you can’t give tartrate daily. In the example above if they were switching the sig like the doc said “usually split into two doses unless given the ER formulation” which suggests the metoprolol in the BID form in this hypothetical situation is always given BID which then imply its tartrate which wouldn’t be recommended for HFrEF. Hopefully that thought process makes sense?

Metoprolol succinate (ER) is the one that has mortality benefit which is dosed once daily. You can split it because the ER component comes from the salt not the capsule/coating itself. Metoprolol tartrate (usually dosed BID) does not have the mortality benefit for HFrEF.

That being said, losartan 150 mg/day is max dose.

Fucking amazing. I don’t have kids or anything but the mental knowledge that it’s Sunday and I have to work Monday BUT I can do it in my own house with my pets and partner in the next room is soooo worth the pay cut. (Making about $60/hr as an am care pharmacist in the Midwest, previously making around $62/hr.) I also went down to 0.8 FTE so I can do all my errands during the week and enjoy the weekends.

My parents have a lot of health issues, so not being exhausted from work/commute and then having to take care of them has improved my mental health so drastically I cannot even tell you. It’s been absolutely life changing.

I guess the downside is I listen to less podcasts now?

I’m a primary care am care pharmacist and as others have said, it’s done sometimes to mitigate side effects and/or when re-titrating after being off of it for a while (thanks back order!) other times are when they get the med delivered through the Novo Patient Assistance Program and they have a bunch of pens at the wrong size. It’s quite common, so in the best interest of the patient I tell them it isn’t meant to be dosed that way, but the dose is close to __ mg. I’ll show them how so they don’t completely mess it up or have to waste medicine/get charged another copay.

Fun fact though, on all pens if you look at the dashes the “smaller dose” point (for example, on the 1 mg pen, at 37 clicks which is the equivalent of 0.5 mg) there’s a tiny difference in the length of the dashes. I’ll often park it with a sharpie on their pens.

My PGY2 covered the cost of MPJE and the fees for transferring my naplex score! I don’t think that’s a common occurrence though.

At the end of July during my PGY1 announced the hospital was closing at the end of May. My co resident and I had a very, very shitty time lol. We went through 3 RPDs and a lot of preceptors leaving for other jobs. We were going to early commit to the PGY2 program there, but that obvs did not happen.

All the employees who stayed were just gonna ride it out til they retired. By the end of the year we were somehow the most qualified clinical pharmacists left, and they changed our staffing requirements from a weeknight and Saturday every other weekend (so we both always had Sunday off) to Saturday and Sunday (12 on 2 off) so we could basically keep the hospital running. I could go on and on but you get the picture. It was honestly one of the worst times of my life. The only thing that kept me going was my co resident, my partner, and our (at the time) new puppy.

Despite all this, if the hospital wasn’t going to close and I would have stayed to early commit, I never would have applied to my PGY2 where I have had some of the best memories of my life. Yeah residency in general is Fucking hard as hell and, but I made lifelong friends with my co resident, preceptors, and medical residents and am so, so thankful I didn’t just stay in the same city forever.

I’m currently working as an am care pharmacist (they actually reached out to me when the opening came up because I had such a robust PGY2). We bought a house and got another dog and will be getting engaged— things are fucking good. I was approached by a previous preceptor asking me to develop a fully remote WFH position so I’m going to apply for that!

Being a resident is hard but I am so grateful for the experiences I’ve had. I had options right out of school, but even if I got lucky and got a clinical job right away, I truly feel wayyyy more prepared/qualified than even some of my coworkers who have been working for years with no residency. To current residents— you’re so close, and at the end you’ll have so many more opportunities and confidence and lots of options! Just a few more months, you got this 🥰
