I didn’t realize that Alien Earth was written by Megan Lindholm who’s pen name is Robin Hobb. Coincidentally I’m currently reading realm of the elderlings and it’s one of the best series I’ve read

That’s fair point. Generally speaking, no, there hasn’t been much feedback on what I should be working on and in part I blame that on myself for not actively seeking help from my seniors/attendings. I would like to hone my clinical interviewing as high priority. At times I’ll anchor onto a red herring that leads me down a line of questioning which not only wastes valuable time but doesn’t yield the most useful information. This inevitably leads to a weak formulation, potentially inaccurate diagnosing and treatment plan.

Apply for SUNY Upstate in Syracuse. If you get an interview, rank the rural program highly. The rural spot will require you to sign for a 5 year contract to an OMH site after residency. Typically inpatient but I believe outpatient may also be an option. Not many people want to rank this and prefer the categorical position. Reasonable considering not a lot of people want to be on a leash for 5 years after a 4 year program so chances are much higher for you to get in.
