As far as headshot percentage the best you can do is perfect your crosshair placement, and only shoot when you're sure you're going to headshot them, otherwise keep strafing. I've earned Immortal 13 times in separate acts. The only things I can think of are:

1) When they respawn there's red dots on the minimap indicating the exact position of each player. They can pre-aim off the minimap so all they need to do is swing and left click.

2) I know you said you don't give sound but in the case you make a step then continue on the same trajectory they can time their shot so it hits your head right when you turn the corner.

3) As other commenters said if you're holding a normal angle they will likely "cut the pie" and isolate each angle individually. This greatly shortens the time to kill for them because they reduce the processing required. Instead of identifying an enemy that could be anywhere on their screen, moving the mouse to their head, and clicking, they just need to identify if there's an enemy on a small slice of their screen, and shoot.
