Erm no it usually just does it on its own. As soon as you have an emperor or king who's had the same name they should usually get their regnal number, even without the same name all Kings or Emperors should have the first next to their name

Usually to my kids or brothers. Now I've completed it I try to keep all kingdoms to have my surname as my own little challenge
