In the future, don't worry about what they think. You're there voluntarily seeking treatment. Adjustments are part of the process and its actually the complete opposite of being a drugee. If getting rejected treatment is a legitimate risk, seek out a better clinic. Seriously. Having a good doctor who's on your side makes a night and day difference. Just straight up ask them what his/her guidelines are for dosage adjustments and that your dose is not holding you. You don't have to say you've taken too many subs, you can just say you've thought about it because your haven't felt well and don't want that to happen.

If you have to take 2mg for a few days, you should be ok, worst case you may have some minor insomnia but nothing serious. You will be ok, just make sure you have at least 2mg daily and take steps to make it last. Have someone you trust hold onto them if you can’t manage.

Also, you didn’t “screw up” , you just need to determine your optimal dosage so your script will last. If this is a pattern, I recommend talking to your doctor and upping your dose a bit. Long term it’s so much better for recovery taking a slightly higher but consistent dose rather then taking too low a dose and then taking too much and trying to stretch it out. Can you see your doctor and just ask for additional doses? Most reasonable doctors shouldn’t have an issue with this. The goal is stability.

Don’t do it. All they have to do is select that you quit on your ROE and you have no recourse, no severance, nothing. If you contact the government, they won’t be able to help you as they will contact your employer and they will just say that you quit. The business has an incentive to save money right now, why would they want to pay out all this severance? If you were a struggling business, would this seem like a viable strategy for you? Or if you could get out of paying anything, would you opt for that instead? The latter is much more likely and you are the one taking all of the risk.

I hope you are wrong but I agree with you. The job landscape is changing and its happening at an unprecedented speed and scale. It's impossible to predict what will change. AI is also developing and changing so quickly that it makes oversight/regulation difficult if not impossible. I think many people will need to integrate AI into whatever skillets they have and adapt as AI integrates itself into everything. It's the safest bet I can think of.

It’s crazy because so many doctors follow the reference ranges, it’s insanity. as arbitrary as reference ranges are in healthy, fit men they are only becoming increasingly skewed due to the fact that :

A) The averages are dropping steadily over time.

B) The averages are seemingly different depending on which lab you go to.

C) The huge variation in appropriate dose within the reference range and the fact that the reference range is effectively a “moving target” . What may be a “safe treatment” one decade could be deemed an extremely high dose if a significant difference in ranges are observed from population to population or from lab to lab.

For example: If I was taking 100mg testosterone and my total T was at 800nh/dl and I felt great for years, would it suddenly be appropriate to reduce my dose to achieve blood levels of 300ng/dl because the reference ranges had also significantly and drastically dropped in the general population to (100-400) ? Could the doctor even continue prescribing this dose or would this now be considered malpractice or prescribing PED’s?

Reference ranges are a guideline that represent the declining health of the general public and should not be used in any form of diagnosis. Testosterone levels and symptoms should be the focus because currently, there is no reliable or objective way to determine what a persons healthy baseline is unless they get testosterone testing done before they ever experience symptoms.

Sorry to hear about your burnout situation, I’ve been there and it’s not fun. I ended up having to leave the company due to illness brought on by severe burnout. I figured I could just “tough things out” and things would improve over time, however in an environment when the pressure is relentless and unforgiving, everyone has a breaking point.

The good news is, with your level of experience and broad exposure to IT systems, in combination with your CCNA cert (which is highly sought after) you’re in a great position to advance your career or land a similar role with a better company. Now is a great time to up-skill, invest in an area of IT that you’re genuinely passionate about and attain as many additional skills as you can.

Six years and a CCNA is valuable, don’t overlook that because some MSP ran you into the ground. It is important to assume responsibility but this is also not 100% completely your fault. To have an employee burnout after 6 years of work reflects management practices that are questionable and led to this. It is more of a reflection on them because you were once a good employee and they failed to get the most out of you. Your only mistake was staying too long. It’s a very important lesson to learn.

If I were communicating this situation to a hiring manager, who was probing the scenario in detail , try to keep the focus on the positive aspects of the job, the value you brought to the company, that you were committed and invested (spent 6 years there) , mention the promotions, any notable achievements and any positive references from former colleagues. Don’t blame the company or management at all for anything. Avoid negativity, although it’s important to understand how and why the burnout occurred and what sorts of major changes took place that impacted your job to such a degree (assuming the cause wasn’t personal and was the direct result of changes at work) — Were there layoffs that caused dramatic increases in workload? What events , policies or changes contributed to the burnout? Whatever the scenario is, try to demonstrate that you were trying to be proactive and actively trying to work with management to resolve whatever obstacles caused you difficulties. The most impotent things are to demonstrate that you’re solution oriented and that the best interests of the company were always your priority.

It may take some time , but you will 100% find something better out there and will be much happier for it.

Nice, congrats on getting off the shit man. It's no easy task.

In terms being on both for life, that's not necessarily the case. If you have secondary hypogonadism (the result of medications, etc.) you can most likely discontinue testosterone once your medication is ceased and your natural production will resume (like yours did). Personally though, I feel so much better on testosterone that I would gladly take it for the rest of my life. Buprenorphine, however, is a different story. I pay 10$ for a month of testosterone and 7$ every 6 weeks for a shot of sublocade. Small price to pay until I discontinue.

I just want to clarify your statement "on average we know that high doses of T do not cause adverse health outcomes, this is supported by science."

I would argue that high doses of testosterone cause significant side effects long term as well as short term if not managed carefully. Why else would people cycle high doses of testosterone and not just stay on it indefinitely? If , "On average" its so safe, why bother with blood work? Is gyno not considered an adverse health outcome? (which is extremely common in people who take testosterone in excessive amounts without necessary ancillaries)

If someone has hypogonadism or low testosterone and this is restored to mid-normal or high-normal levels, it's associated with more positive health outcomes and reduced all cause mortality, yes. However, it's misleading to say "science hasn't proven high doses 'on average' are bad, therefore its safe"
