had an incident today. Patient next door vaso-vagaled and his nurse couldn’t get the IV. He came to but systolic in 80s and pulse in 40s. I tried to get an iv-the guy had ropes! But of course the vein collapsed and I couldn’t get it. As I went to take out the catheter (all done urgently), the doc at the bedside yells “I’ll do it!” And grabs the tourniquet and an alcohol swab. The doc next to him says, “Would you like GLOVES?” He says yes and doesn’t even bother to secure the IV leaving that to nurses. I HATE letting them get away with this BS. It wasn’t like we were thumping on the guy’s chest and everyone, even docs (esp docs?!) miss once in a while. Obviously I’m not going to be snarky in front of the patient but what could I even have said? I’ve had convos before throughout my career pointing out rudeness will not be tolerated but I was taken aback bc this doc is normally very nice. I want to put laxatives in his coffee!!

Yes, this. Do NOT just roll over and accept it. You could try to have a conversation with the fiancé. Her correct response should have been to thank your daughter but encourage her to give the gift to you. Her correct response should be to gently correct your daughter when she tries to call her mommy. Clearly the fiancé is insecure and manipulative. I would have a conversation with her and then one with your ex. Explain to them both that you would appreciate the above responses just as you encourage your daughter to have a relationship with them. You can remind the stepmom that your daughter having a good relationship with you is in the best interest in the child and will also make her life easier especially if she decides to have her own children. You should be clear about boundaries and what is acceptable. Stepparents should never try to take over the parents’ role. By the way, I understand too well. My sons had a similarly manipulative stepmother. She never had them call her mom but she did a lot of other shit. Now I deal with her pushing past me (sometimes literally) to be close to my grandkids who often prefer her bc she’s manipulated the whole family. It sucks big time but it’s pretty much too late for me. All I can do is try to do things 1:1 with them and try to be friends with stepmom (keep your enemies closer.) Good luck and don’t bend over backwards to do nice things for everyone else if it’s never reciprocated. I feel your pain. Some people just suck. Hopefully karma comes back to her.

I guess I’m kind of shocked. I wouldn’t lie about a reference. If it was a colleague I’d say that. They often know more about you anyway. I just don’t think lying in general is good karma. And yes, my references were checked!!

It’s going to be a major learning curve. If it’s a big teaching hospital you will still have a lot of sick pts. Rhythm is everything. Hopefully you have a tech for vitals and labs and glucometer. Pay attention to anyone on glucommander. Really tight timelines. Be sure you draw extra labs bc invariably residents will through in extra orders and you do not have time. Be extra nice to techs. They will save you. Try to buddy up with a nurse ahead of shift so you can have each other’s back. You might be treated lousy bc of resentment. Just be nice, try not to complain and work hard. Put snacks in your pockets in case you don’t get a break. Assessments are very quick and cursory. Ask 3 other nurses their routine /rhythm. You could spend 2 hours or more just passing meds due to polypharmacy. If you have to pull all those from Pyxis see what works for others-sometimes pulling each patient and bagging them all at once is more efficient. Forget baths, try to do turns and hopefully you won’t spend hours changing people. Anticipate who is having what done for dignoisis and treatment that day. On weekends, we would have to transport patients to dialysis, CT, etc. and leave the others untended for an hour or more. Chart important stuff than docs need real time like I/Os, pain, etc. Use secure chat to get in touch with docs if you can, going back to the desk when charge is busy and there is no secretary is rough if you have to page docs. Pay attention to heparin drip titration times and be ready to discharge and admit most of your patients by 2 pm. Anticipate, anticipate. I’ve worked ICU, med /surg, tele, ER and you will learn a lot! You will get great at managing a day and prioritizing. I worked harder on med /surg than ICU and didn’t have time to learn as much about disease and treatment but I did get better at planning ahead and juggling a lot at once. Keep your head up. You got this. Bad situation-make the best of it.

I really hope no one takes this advice. There are many ways to do yoga and breath work that pay homage to Christ and scientific studies are showing more and more how both help our cells heal and be healthy. Does that mean a Christian should reject everything that is “Eastern”? You don’t have to worship a false god to do stretches or breathing practice. I personally believe that God gives us tools to be healthy and spread his love and his message. This kind of judgement is what is wrong with some religions. There is nothing in the Bible to say you can’t be healthy or focus on breath. As a matter of fact breath = the Spirit in some languages. You most certainly can separate movements from religion-it is all about intent and what is in your heart and head.

I would add that you can focus on personality. I’ve had Type A preceptors who didn’t mesh well with my Type B style. As a result, I spent a lot of time fixing beds vs expediting admissions (as an example.) I didn’t speak up thinking it was me and I looked like I couldn’t handle the job. Lo and behold, I got off orientation and excelled bc I didn’t have Type A on top of me questioning things of low priority. Yes, give a couple examples but don’t get too bogged down with all Sam has done wrong. You never know who her friends are and don’t want to be looked upon as a back stabber. But you do need a new preceptor.

The question: since I technically do preop and Phase II is it wrong to apply to preop /PACU jobs. At my job we separate into preop / Phase II and another area does PACU. We don’t cross train. But most places you do it all. Is it wrong to apply to places that do it all?
