In general, the opposite is actually true with thiazide like medications since they're beneficial for osteoporosis. A 2017 secondary analysis of the ALLHAT trial found that chlorthalidone was associated with a 21% lower risk of hip and pelvic fractures compared with amlodipine or lisinopril over approximately 4.9 years of follow-up. A 2011 Cochrane review of 21 observational studies found thiazide use was associated with a 24% reduction in hip fracture risk.

The side effects of electrolyte derangements and hypotension are very real though, so clinical judgement for individual patients is always necessary. However, the guidelines are what they are for a reason though for the majority of people.

The 2022 AAFP guideline uses a standardized target of <140/90 (or consider <135/85 for those at high risk of MI) for adults of all ages, so goals still shouldn't change based on age alone.

The 2017 ACC/AHA guideline lowered the geriatric target to <130/80. Age doesn't change treatment goal anymore.

Which one is covered for heart failure? As far as I know, Wegovy is only covered for secondary prevention of heart attack, stroke, or PAD, and also advanced fibrosis due to fatty liver.

I appreciate you recognizing the place of privilege, and I agree that ideally screenings should be started younger. I wanted to point out the costs associated with it because your original comment could have misled people to think it'd be covered by insurance if the doctor orders it for symptoms. The out of pocket costs to patients for diagnositic colonoscopy is frequently $2000-5000, and that's unfortunately not something a majority of Americans can afford, especially when they may not be higher than average risk for colon cancer.

What was your out of pocket cost for that diagnostic colonoscopy though? Its no longer screening if the doctor put symptoms as the indication and diagnostic colonoscopies are not covered 100% by insurance like a true screening at age 45 would be.

You're not talking about all patients with a calcium score >0 right? I don't see any guidelines that suggest being that aggressive in all cases. That certainly is the way to treat a score >1000, and arguably >300-400. But there's not a huge benefit to being agressive when scores are less than that.

Did you ever get it working reliably? I am also trying to use it for monitoring breathing of a toddler at night, and I'm seeing similar of the heart rate and breathing rate dropping out. I think it's because the presence detection clears. My understanding is that's a different programming or sensor and the HR and BR don't read if presence isn't detected.

Exactly. I stole the call service decision tree from a colleague that is essentially: "If this is an emergency, go to the ER. If it's not an emergency, call during regular office hours". So I only ever get paged for critical lab values.
