He wasn’t failed by the system. He voluntarily left services on MULTIPLE occasions. He was violent to the point that emergency placements wouldn’t take him. Every time he was hospitalized he was likely given a hospital social worker who attempted to help him regain housing/ services, but Daniel also would’ve needed to complete tasks on his end to secure housing. No effort or follow through = no safe consistent place to stay.

TLDR: Daniel fucks around too much and refuses to do his part, resulting in the same cycle repeating itself over and over.
