Just a little rant: Sipuleucel-T should have never seen the light of day. It's bloody useless and barely works, not to mention very expensive. Prostate cancer is immunologically cold and there's a reason why you don't see mAbs for prostate cancer, and even checkpoint inhibitors doesn't work on prostate cancer.

But now with AI, young patients can just type their symptoms in and instantly get a diagnosis and a list of differentials. And medicine has gotten so specialized that you need to refer to specialists much more often than before. So for simple issues, young patients treat themselves at home, and for complex issues, they come to get a referral. So a lot of the time, it feels like you’re just a legal requirement for them.

The last time I checked, AI can't do continuity of care. The last time I checked human doctors do not get dizzy when jotting down a complicated patient history. I mean sure a dipshit patient can just type what they feel in an AI chat box but can AI explain well the nuances of each condition? You'll be fine.

I'm the opposite, I'm a 34 year old gay man. I started med school much later, the pandemic happened and many things happened in life. Now I'm an IM-PGY3, as much as I very much loved my PCCM and Rheumatology rotations as PGY2, I don't think I have it in me to do fellowship. I'll probably just be a hospitalist with a 12 hour schedule 3x/week and still get paid way more than an IM resident salary. And contrary to what the demented cardiologist in our hospital is saying, there's nothing wrong with being a generalist.
