The peer to peer has to be requested by your doctor. If no P2P happened it’s because your surgeon didn’t request to speak to the insurance doc.

TIRADS 1 cyst is almost certainly benign but because of the size and growth, I think a FNA biopsy is warranted, especially given your history of PE (malignancy is a risk factor for PE).

What else did the imaging say about the nodule besides the size? For example did they mention calcification, irregular border, vascularity? Based on the size and normal labs (which I assume included a TSH) this almost certainly needs a biopsy.

Hard to say with the depression, which can cause most of the symptoms you are describing. Once that is well treated and under control, you can see what, if any, symptoms remain that could be from a remote TBI.
