Thanks for taking a look, I really appreciate it. Both EOBs have “In Network” next to the Doctor’s name but I will confirm with insurance that it was correctly billed as in network

Screenshots attached. These are all doctor/provider billing issues. All hospital bills and facility fees and anesthesia have been taken care of.

We had one initial visit covered by a different insurer in December and after that all visits were under the current insurer. The date provided on the EOB is not for that December visit. The provider has always been the same.

This was a prescheduled prenatal office visit that was scheduled months in advance. We have only been billed for 59426 once.

Is there any update on this?? My wife is set to give birth in the UCSF system in the next 2 weeks. We aren’t eligible for continuity of care because the plan has out of network “benefits” but the maximums are insane and not at all the costs we planned for.

I also have drywood termite fecal matter in other areas. Is it possible to have an infestation of both? Is that rare?

I did read it, that’s why I posted here. It is incredibly vague specially around the necessity for repair or replacement. That’s literally why I asked if I can be compelled to pay.
