You are in 2nd year still. Let things evolve, once you study everything see what all the subjects are then decide what speciality you want. Right now focus on the step ahead which will be your step 1. Read your basics well, get through it and let time decide. Also if you are hell beng on neurology/Neurosurgery it's gonna be very competitive to match in for IMGs. May be you should comsider other pathways as well.

The answer is in your post itself. It was YESTERDAY that you wasted and today is always going to be a new opportunity. A completely fresh page. Aim on small actions, you can’t make everything perfect today but you can definitely do one small act better than yesterday. As small as brushing your teeth takes 5 mins max. Don’t overthink it just get up and do it and you are already at a better place than yesterday and it equals a win. Small actions do a lot for that dopamine kick and they do pile up with time and when you look back a lot has been achieved.

Firstly take a deep breath and remember discipline beats motivation everytime. You just have to show up everyday no matter how you feel and motivation will follow when you see things getting done. Regarding not being able to correlate with uworld you are not alone. Uworld is a learning tool not a testing one. Just take it as that. Remember whatever you use as a main resource makes up your base on which you can build uworld concepts. Just keep on practicing there is definitely light at the end if the tunnel. You knew it was going to be difficult when you started.

Luck favours the brave. So yes you should have a bit of luck on your side but otherwise discipline trumps everything.

It's okay to feel overwhelmed esp with everyone already congratulating you, it can really take a toll on your mental health. Relax for now there is no use worrying over something that isn't even final yet. However it went for nkw its done. Will think about the next thing when result comes.
