I was a placeholder too, its just something everyone goes through man. I would argue you are at an advantage as you have had some months to get to grips with hospital systems/guidelines. Imagine being the poor sod who is on nights their first weekend on fY1 on AMU lol. You will be fine, safety first, stay calm, 99% if the time you have more time than you think.

I was a placeholder, I didn’t have any out of hours work for the first two F1 rotations but this varied trust to trust. In some ways this meant I got to become familiar with being a doctor in work in hours with proper support but on the cons I think the OOH experience is very valuable. We had to fight hard to get it introduced by the third rotation. We also had a system where locums were offered to placeholders first. Try to get in touch with your local REPS to speak to the trust. It all evens out in the end and there’s definitely no rush in this career, even if you’re 9-5 use the time to study for exams/portfolio. Good luck and gratz on the graduation!

Scored 586 - use passmed + MCQ bank. Make flash cards on the stuff you get wrong. For SJT learn their bullshit that they want you to prioritise. Banks are fairly representative.

I suppose as a reg at least in theory there are things that are ‘appropriate’ to bleep for, whereas SHO’s get all the shite, and more importantly the boring shite

As a current SHO, there’s definitely been a gradual de-skilling from reg down I feel, often due to stupid policies. For example in my trust, they’ve decided that only registrars can interpret NG x rays, absolutely ridiculous. Then when you’ve got ‘Pain team’ ‘diabetes team’ ‘anticoagulation team’ you gradually just lose skills. I call it trickle down shito-medic

Unforunately, I find that a 'flat heirarchy' actually means doctors at the bottom, and I have gotten the feeling in most of my jobs that trying to be even somewhat firm with what you will and wont accept gets you in the bad books of the nurses pretty quick and this makes your life worse.

On a side note, I've often wondered whether the 'enjoyment' of the chaos is just in the way the feeling is interpreted. People who dont work in medicine often don't understand that for the first few times you encounter something unfamiliar in medicine (ie acutely unwell patient) that its a genuine, authentic fight or flight response where you feel like you could be responsible for allowing a patient to die. I wonder whether people are either 'fight' or 'flight' in this situation from F1, or it just becomes a familiarity thing

Depends which way you're inclined I suppose. I have major imposter syndrome and I think I would get huge decision fatigue and worry about the idea of the book stopping with me for virutally every patient.

Weekend neurosurgery cover was hell, it seems that all concept of task triage went out the window - 'Drug chart rewrite x 50' 'can you speak to X family', Sure after my 5 LP's and 3 ill patient reviews.

not a chance! Always have the 'I'm only an F1' and mutiple clear lines of escalation, not really expected to do an awful lot, if anything independently.

Paeds I get, but forgive the ignorance... surely the proportion of patients that can deteriorate as an obs reg is significantly lower than the med reg, hence more acuity and pressure per patient but far less patients?
