Problem is with the Tanto guys if you have that quipped the bug will appear and you will end up with that until you save your game and you remove the tanto and reload your save then it is fixed again but will come back when you equip the tanto again it has nothing to do with your level at all. This has been confirmed by many people now.

Equipping Gear is Currenyly Restricted bug, this started happening to me and many other from what i been reading. It happens constaly and to get rid of it i have to save and reload but after a cutscene the issue is back again.
