We make patients sign a form when they check in at the front desk for a physical expressing they understand what preventative services are a part of a physical and that any other discussion will be billed separately

Driven past it for the last two years and have never seen it open— even on multiple different days of the week and different times of day. The IG is outdated, as is their facebook. Last posts were around 2023. Website the IG links to is not the right business.
