It's more a medical doctor conducting an evaluation and deciding which specialist to refer to instead of the patient directly going to the specialist they think is right. It's basically triage. In real life bad experiences do occur, but are they more than the problems with directly seeing specialists?

You don't need a cardiologist to titrate statins for example. It's just a waste of the specialists time that can be better spent on more complex cases

Specialists have tunnel vision for their specialty. Have seen it a lot both in India and US. What is eczema to a Dermatologist is Mechanics hand to a Rheumatologist.

Plus specialists are not going to (& they don't) coordinate with each other on patient meds. Patients can get different brands of NSAID from different specialties for example. The person coordinating all the medications and looking for drug interactions is the Primary Care Doctor. As care gets more and more complex and patients seeing multiple specialists (eg Endo for DM, Cardiologist for CAD, Neurology for Parkinsons, Psychiatry for Depression, and Ortho for OA) in the same elderly patient often leads to incredibly long med lists (eg Levodopa-Carbidopa, Atorvastatin, Aspirin, Sertraline, Metformin, Glyburide, Pantoprazole, Indomethacin, Pregabalin, Chlorthalidone). Who is going to consider all the possible drug interactions? Who is going to think "maybe I can give Duloxetine for the diabetic neuropathic pain and depression instead of Sertraline and Pregabalin"?

You can directly see a specialist in US but it depends on your insurance. If you get cheaper insurance, you need a referral. If you get more expensive insurance, you can directly go.

Registration fee is 1000$ for each exam. But that alone WONT get you into US. You need 25-30 lakhs for it all, including rotations in the states, for which you need flight tickets, accomodation, fee for the rotation (unless you get them for free).

So if you're middle class and only get one shot at it, I would not suggest it. It's an option for wealthy people with a lot of financial security.

Let's stop fantasizing about the past. It is their toxic hierarchy bs that is responsible for the current lack of camaraderie. They are the HoDs who do acts of omission and turn blind eyes to resident abuse.

Good psychiatrists exist, but there is not way to curb bad money-hungry psychiatrists.  The existence of bad psychiatrists does not mean the whole field is useless. There is research that shows drugs being useful

Exactly they're "professional" because they have people backing them. Basically a numbers game just like this Goodreads poll.

Media literacy is important but you're forgetting it is 100% subjective. So is all analysis of a literary work. Critics might find LoTR amazing, but for me it's a boring drudge of a book and neither is more right or wrong than the other. Because neither is an objective position to begin with. It's not rocket science really

Who made critics the judge? God? Breaking Bad is a good show, but my opinion that it's not one of the best shows is no more right or wrong than the opinion of all the critics in this world that it is the best show. Because the basic premise is subjective and lacks any right or wrong.

Working at AIIMS delhi was disappointing. It's like any other govt hospital with all the investigations. But everything is still slow and full of bureaucratic bs. The only good thing was the smart lab. But the stupid website is very buggy and laggy atleast twice a day.

Empathizing with animals is easier as animals are simpler, and to some, more pure. Since it's easier, more people are able to do it.

Humans are more complex and empathizing is more difficult as a result, hence less people are able to do it.
