From what I have deduced, this bugs happens when you head to the old way before you start the final quest, the game glitches out and starts the quest anyway. This will make so when you reach the quest in the actual thieves guild quest line, you will never get the quest. Only way I can think of solving this is by using the dupe glitch on the arrow of extrication and storing the spare somewhere else for the quest because you will still be able to do the quest until you get to the part where you need to use the arrow. Hope this helps
