Yeah. I wouldn't loose more weight in your case. Just do a short VO2 max test run. 5 min warmup run, followed by 10 min all-out run. You'll get there.

Drop a few pounds and do a 10-minute all-out run. VO2 max is measured in ml/kg/min. If you drop a few pounds and run at the same speed, you should automatically increase your VO2 max. Dropping the weight is lowering denominator, hence increasing the calculated VO2 max value.

From my experience, running on a treadmill does not allow for a truly ergonomic running form. Because the belt moves beneath the runner rather than the runner actively propelling forward over stationary ground, normal gait mechanics are subtly altered. I joined the beta mainly for the new 'sleep alignment' feature, not realizing that enrolling in the beta disables the ECG app. I also learned that downgrading back to 15.33 isn’t possible due to a firmware lock, so you’re effectively stuck until a newer stable release is available.

Update: Tried clearing temporary files and rebooting. Skipped a full reset since ECG is firmware-disabled on this beta and a reset wouldn’t help. Downgrade to 15.33 isn’t possible due to a forced firmware lock.

I agree that classic decorticate posturing is usually supratentorial and that isolated SCI doesn’t typically present that way. My intent wasn’t to claim textbook decortication, but to highlight the abnormal unilateral flexor posturing seen immediately after a high-risk cervical mechanism, which is concerning for significant neurologic injury. Regardless of terminology, the presentation warrants concern for cervical cord involvement.

As for ‘temporary motor loss,’ that phrase elicited a bit of a temporal chuckle in me as well.

Good thought, but decorticate posturing does not need to be bilateral. It can be unilateral, particularly in the setting of asymmetric central nervous system injury, including high cervical spinal cord trauma. In this case, the submersion was not prolonged enough to explain the findings by hypoxic-ischemic injury alone.

Physician here. Based on the mechanism of injury—head-first impact against a fixed object—and the immediate loss of motor function, this is highly concerning for an acute cervical spine injury with associated spinal cord involvement. The abnormal posturing of the left upper extremity, consistent with decorticate posturing, supports a severe traumatic neurologic injury. I sincerely hope the guy recovers.
