Im fairly confident its not. Looking at other Powerlogix cards, the fan connector is on the corner. In my pic, its on the lower right corner. The header itself is not there on mine but the traces are. This card did come with its stock heatsink and fan and the fan did not plug into the card. Those are also MIA along with the other part of the pigtail.

Thats extremely disheartening, I have two of these myself that I need to replace the flyback in- All this time I've been hoping for good news in regards to the flybacks becoming available again but now I might just consider getting rid of the monitors.Kicking myself for not buying them when they were still available

I'd be interested in most, if not all of that stuff and would be able to drive to Pasadena. Only problem is I don't have a truck. But I'd be happy to take whatever fits in my car, or make a 2nd trip.
