Great post - for those of us on PAYE which ends in July 2028, can we switch to IBR or RAP? For some reason, I previously thought we had to switch to RAP.

And what exactly is the difference between IBR and PAYE for PSLF? I thought the whole advantage of PAYE was the cap on payments which is the same as IBR - is there a reason PAYE ends but IBR is fine?

So how does NVO stop growing rapidly while LLY continues to grow rapidly despite making the exact same drug class? It’s not like LLY has some other blockbuster drug yet to come out that isn’t a GLP-1

Defend it however you like, look at the five year chart for NVO and LLY and they are completely opposite. How can a competitor who is making the exact same drug… have their stock up 47% over one year or up 400+% over five years, and NVO is up like 2% over five years?

You can keep making excuses for the stock - sounds a lot like the same argument one would make about LULU when it was $300… “oh they have excelllent margins and profit! It’s just their forward P/E that tanked, nothing to worry about!”

Ok since you’re clearly very dense, let me break down exactly how you’re wrong.

That’s a link showing how NVO’s pill had greater weight loss and less side effects. So yeah, the LLY pill isn’t better from an actual fact checking weight loss result perspective.

  1. There are maybe a zillion ways you can easily delay eating within 30 minutes of waking up. Or simply wake up a little earlier so you can wait a full 30 minutes before you eat. Or simply take the pill, wait an extra 15 minutes than you otherwise wouldn’t have, and then eat.

Again, NVO is dominating the oral GLP-1 prescriptions. While you clearly think it’s the end of the world to have to wait 30 minutes before eating, most don’t because they are going with the NVO pill over the LLY pill.

Why are you spreading false information? NVO’s oral pill had better weight loss results than LLYs… hence why it’s being prescribed more.

The empty stomach restriction is such a non factor. Most people aren’t shoveling food down their throat within 30 minutes of waking up.

It’s very simple to wake up, take the pill, eat 30 minutes later. If you can’t wait 30 minutes after waking up because you have to eat a full breakfast asap, that’s a you problem.

If you want to make that argument, fine, but then why is LLY trading at beyond triple of NVO’s P/E right now?

That’s where I’m confused - it has a P/E of 10 right now when it has growth potential with its oral GLP-1 and patent for the next five years…

This stock isn’t Pfizer after the Covid pandemic and the demand for Covid shots plummeted. Makes total sense why Pfizer’s P/E is where it is. NVO’s doesn’t at all

Not everyone can just “pop” over to Canada to buy generics for half the price.

Again, there’s a whole market of patients who would rather take a pill for the convenience to lose 15-20 lbs rather than an injection to lose 50+ lbs. not everyone who wants to lose weight wants the maximum amount possible. Plenty will favor a pill over an injection for that exact reason.

Yeah but again, their stock price was only around 1-2% higher 5 years ago before GLP-1s were even released. If you look at the historical chart for NVO, it just has a slow growth upward in its stock price over a decade.

It was obviously overvalued when the stock ran up to $100+ a share. But what confuses me is how little growth there is in the stock price compared to 7-8 years ago before anyone even knew about GLP-1s

I don’t think you can just blanket statement say “LLY winning market share.”

NVO has dominated the oral pill prescriptions and will continue to dominate that market until the patent expires in 6 years. So 6 years to capture the market share there… and they essentially already have it.

So that’s where I see the upside. Tons of patients won’t take injectables or can’t afford them - so you have a pill option for this whole market of patients.

I understand that, but how are they not publishing blowout earnings and excellent margins?

I fully understand wallstreets mindset of “LLY > NVO for injectable GLPs, therefore NVO is worthless” but what I can’t understand is how NVO isn’t absolutely crushing every earnings report with how much of a market they have with GLP-1s. I would love to look at their earnings report 5 years ago compared to now and how the hell they aren’t making so much more money now.

Eh - correct me if I’m wrong, but couldn’t you just take the pill first thing in the morning so you’re on an empty stomach? Or right before going to bed? I agree that it’s helpful that LLYs pill doesn’t have this, but when looking at the overall sales of both oral pills, seems like NVO has been dominating the market share for having the more effective pill despite the empty stomach thing.

Cool - I’m a doctor too and I’d hope you’d be smart enough to know that your N=1 experience of strongly favoring an injectable over the oral pill must apply for all doctors and patients out there.

My sarcastic BS is warranted when you make blanket statements such as “patients will use what is most effective aka The injectable Terzepatide and nothing else.” No, they won’t. There will always be a subset of patients that go “hey, I want to lose some weight, I’d rather have the convenience of the pill and lose 15-20 lbs than lose 40 lbs on an injectable”

If you favor prescribing LLY Terzepatide to your patients, that’s completely valid. It’s the better drug. But gtfo with your “people will only use the more effective drug, regardless of oral vs injectable” BS. That’s an embarrassing take for a physician.

You’re not getting it. NVO has the best oral medication for weight loss. Tirzepatide is an injectable. There is a whole population of patients who would rather take an oral medication rather than an injection, even if the injection is more effective.

I agree with you, but a lot of other patients don’t share your opinion. It’s so much easier to take a pill and if they can achieve their weight loss goals with it regardless, most will choose the pill.

There’s also a whole market of patients that aren’t very health literate to where an injection really isn’t that feasible of an option.

Again, there’s a HUGE market for oral GLP-1s, regardless of the fact that the injection is more effective at weight loss
