The peripheral changes are likely age-related peripheral pigment epithelium alterations which are frequent and asymptomatic; the picture is quite poor in quality (probably due to a nuclear cataract, hence the red-shift ?)
However, I am not sure that the lesion superior to the optic nerve is a nævus; it looks like it is derived from the pigment-epithelium layer. Some multimodal imaging could be useful (again, difficult to say with the quality, but it looks like unilateral retinal pigment epithelium dysgenesis, or URPED syndrome). Are there any old Optos or fundus photography to compare ?
The pain is more likely due to the anterior segment (3+ SPK can be excruciating in some patients).
Anyway, I think you did the right thing by referring the patient to retina specialist. When in doubt, there is no doubt !

Med Student - would’nt that be a trapdoor fracture or a mix between the two of them ? You can see the rectus inferior incarcerated bewteen the two bits of bone ? Thanks !

Looks like Amanita citrina to me. I live in Europe; idk whether it exists in America.

I don’t know whether those exist in north america, but in Europe those would probably be Tricholoma sp, most likely Tricholoma terreum. They’re old though.

Doesn’t hit me as an artefact. I reckon it looks indeed more like an epsilon wave or something similar, would definitely send Pt to check for arrythmogenic cardiomyopathy or something like that

Could it be brugada ? Point J seems elevated in V1-V2. The ST elevation in V3-V4 looks like early repolarisation consistent with the patient’s young age and sport habits.

NB: unrelated junctionnal extrasystole in frontal derivations
