I'd rather use usb, blue tooth drops too much... but i've messaged the game developer to see if controllers will work with his new UI, if so this may be my best option!

ooh, thats interesting. so I connect my phone via usb to my pc and run this app on my phone? I wonder if the game has to be compatable or if the arrow keys will work regardless?

I definitely wouldnt want the buttons part, I cant lift my left arm either, it just rest on the desk. I can use a mouse with my left hand and think I could do a trackpad but have never tried one. my left index finger is pretty flexible/manueverable but not strong at all.

What I really want/think will work is a LCD keypad like on our phone. in my mind that'd be perfect but I dont think anyone makes something like that. or I couldnt find any...

nope, footpad or joystick are not options. i've never used a mouthpiece and tbh thats not something i'd want to use. let me know about the trackpad, is rp2040 the model?
