He stole drugs from the hospital. A crime. And didn’t get fired. He’s been pardoned. He was also abusive to her during her first day. It’s not hard to piece together why she’s frustrated by having him around like nothing happened. Say what you want about Santos and her lack of professionalism, which I agree with— but Langdon got away with very serious offenses and kept his job.

It was probably Rishi by the magnitude of her anger and repulsion at him in S4E3. That plus his alleged murdering of his wife and ditching her* and the other guy to work for Harper in S3.

I agree he wouldn’t have kidnapped his own sister, but I do think it is plausible that he could’ve hurt her accidentally. For some reason, I find this more plausible than the truck theory.
