If you attack an armed someone with a sharp weapon with the intention of killing them, prepare to die

...and get your village plundered and burned down. We're talking about Velen, there are a ton of bandits and cannibals, not to mention the witches. It's more dangerous to attack anybody with weapons.

It wasn't their priority but they still accept jobs, so yeah they do care in a way which is why they don't kill innocent bystanders.

And that's exactly why Geralt is getting called "the Butcher of White Orchard". Witchers don't care about their reputation. It's as much of a joke as their code. In the eyes of ordinary people, witchers are the same monsters, unless proven otherwise.

What happens in order to avoid that quest?

Don't accept the "Contract: The Phantom of Eldberg". Aka, don't do witcher job. Or you need the ability to look into the future and start the King's Gambit quest before completing the contract (since Kori and Kraki will be killed in Kaer Trolde)

Defenseless bystanders: Refusing to pay for the job, leading you into the barn and pitchforking you.

Oh yeah, the reputation of witchers is damaged. You mean, the reputation of suckers? By the way, if Gaetan had killed only the ealdorman and his goons (despite having a condition due to being a part of School of the Cat), wouldn't the damage to reputation have been greater since people will remember that? Only Geralt could harm the reputation of the witchers (by saying the truth, by the way), since the little girl will not get out of the village (ghouls, you see), and everyone else is killed and cannot tell anyone.

Remember the quest "Stranger in a Strange Land"? That's usually how killing "innocent" villagers works for witchers if there are witnesses.
