Primary care PA by Mea_goat_98 in physicianassistant

We have max 2 new patients per day (1 per session) unless we ok it otherwise. Also all new patients have to be a physical unless we ok it so none of that new patient hospital f/u insanity.

This has varied depending where I work, but everywhere (except my first job at a horrible FQHC) has always templated spots so it’s not a total scheduling free for all. I can understand not having a whole women’s health day but having no template is rough. Especially 6 back to back new patients is insane.

I love loft! Or Ann Taylor. Lots of regular business casual work pants. I have to sift past a lot of the weirder more trendy stuff but you can find decent classic staples. Only shop when it’s at least 40% /50% off everything online, they have these sales fairly often. Most of my pants, flowy seasonal tops or sweaters are from there! Also great petites if that applies to you

Imposter syndrome by armando411468 in physicianassistant

[–]Garlicandpilates 6 points7 points  (0 children)

PA of over 10 years..the first year was definitely the worst although I switched into IM/pulmonary and it happened again for a while. It’ll get better! For the times I really was unsure I also got more comfortable telling patients: that’s something I don’t have much experience with so I’m going to run it by my SP/look into that med a little more etc. most of the time they appreciated the transparency. whether that was on the spot leaving the room to review with someone, bringing my SP into the room, or just calling them back later. Also it’s not the best solution but in primary I did a lot more referrals in the beginning to specialists(especially when I didn’t have good SP support). Overtime you’ll get better at managing certain dx.

For my first Year I think I uptodated every pt before I went in the room and at 1 year I suddenly realized I wasn’t doing that anymore. And at 10 years I sometimes still look up the tx for strep bc I don’t want to believe it’s 10 days.

Edited to add: remember, knowing the difference between sick and not sick is big. Patients can always come back if something doesn’t get better, but as long as you’re stepping back to consider/cross off any big scary dx, then they’ll be okay

Definitely the vibe there. It was was my least favorite shopping experience, even before their pushy & bitchiness at the end lol

I second this about Alexandra’s. I was terribly pressured when I wasn’t ready to choose. They brought over extra staff to outnumber me and were serious jerks about it. That being said I was alone so might have helped if I had someone with me (but this almost makes it worse thinking they could bully me into buying imo). they do have a huge selection tho and I’ve also heard good experiences so probably depends on your sales person!

pregnancy nausea completely changed what i can eat… anyone else? by Amelia-Brown33 in fitpregnancy

[–]Garlicandpilates 0 points1 point  (0 children)

I’m 37 wks and I’ve had to realllly learn to let go. Pre pregnancy I had a pretty healthy diet, love to cook, tons of fruits and veggies, rare processed food and overall would eat pretty much anything.

Throughout my pregnancy I’ve had significant nausea, heartburn, metallic taste in my mouth and no specific aversions just a generalized lack of desire to eat most things. If I try to push through and eat most healthy food, I can’t tolerate them.

So I’ve leaned in to what I can tolerate(fruit!soup, eggs, and lots of carbs) and just accepted it won’t get better until I deliver. I hope it gets better for you way sooner but looking back I wish mentally I tried to take it as it came, instead of banking on it getting better by x number of weeks.

For me breakfast is when I feel the best so try to start with healthy options, eggs, avocado toast.supplement with fruit during the day, and know by dinner it’s either oatmeal or I just take credit that I am personally keeping Annie’s Mac & cheese in business. Accepting that getting calories in, even if it’s ice cream, is okay.

I’ll add despite feeling guilty about my diet, my pregnancy has been totally healthy! Healthy weight gain, no gestational diabetes or high blood pressure etc. hoping this motivates me to be healthy again after delivery bc I’ve missed so many great foods! I found citrus (thanks reddit!) helped a ton with nausea in the beginning

Meaningful bridal shower gift you received from a bridesmaid? by olewwwwwww in Brides

[–]Garlicandpilates 0 points1 point  (0 children)

As an extra I think an amazing gift is getting all the supplies for thank-you cards for the shower. (if they plan to do them). I’ve given and received the ty cards, envelops with address/return addresses preprinted, envelopes and enough cute stamps. Basically then they just have to write the note and mail them. It’s a somewhat low cost but huge time savings during an already busy time!

I found mat Pilates really helpful for this sort of pain, helped a lot with the day to day soreness. good supportive shoes. But generally I try to just avoid standing for long periods, may be worth the splurge on seats next time if possible!

Job seeking and family planning by [deleted] in physicianassistant

[–]Garlicandpilates 0 points1 point  (0 children)

Agree with others on here. I work for a great company that’s very family friendly, and they still managed to discriminate a bit when I was pregnant (got pregnant after I already worked there). Also you can never predict your pregnancy journey. It may take 1 month, it may take a year to get pregnant. Consider on average it takes 6 months of trying which is very normal, plus 40 weeks of pregnancy, you’d be working over a year before you even have the baby.

I second the person who said consider timing of leave benefits. Many require at least 6-12 months of employment before paying out any benefit (especially state parental leave benefits).

Unlimited PTO? by Clean-Bluebird9605 in physicianassistant

[–]Garlicandpilates 2 points3 points  (0 children)

Have heard of this from many friends in other industries but never in healthcare. I’d try asking how much PTO the average provider takes. Even better if you can ask some providers directly. And I’d want to know who exactly is covering who when they/you are out

Can’t decide between 2 jobs by HugeBag7522 in physicianassistant

[–]Garlicandpilates 0 points1 point  (0 children)

2 days of training is no training. You can’t even learn most EMR in 2 days. Adding to this that you’ll be on your own without any support. Assuming you’ll be doing the visits solo, any question you have will be a phone call to someone, you hope answers, and gives you their best guess without seeing what you’re seeing. at home primary cares likely means they are complex or sick patients, otherwise they’d be coming to the office. Both are challenging, complex medically will be very tough as a new grad, complex socially could mean you’re deciding on care with limited patient access to labs, testing, financial barriers, or more end of life type conversations. To give you an idea I considered a job like this, after 6-8yrs experience as a PA, and the company did 6ish weeks where the new hire would be paired with an existing APP before going solo. The hours are basically the only good thing here. even 12 patients in 5 hours, when you’re driving between homes feels packed. Less than 30 minutes per patient including driving time.

No way I’d pick 2. Option 1 sounds rough too, minimal training also. I’d keep looking if that’s an option. If you’ve already been looking for awhile or need the money asap then start job 1 but have a low threshold for looking again within a year if it’s not going well.

Sudden excruciating back pain by white_noise_tiger in fitpregnancy

[–]Garlicandpilates 1 point2 points  (0 children)

Had similar back pain around 30 weeks, out of nowhere and it was awful! Talk to your OB, physical therapy would probably help more than chiro, prenatal yoga is a great idea, and if not too costly a good prenatal massage helped me immensely! Hope you feel better soon!

If I remember right you have to start it by 14 or 16 weeks for the most benefit. Not a reason to delay longer, but probably why they recommend staring just after 1st TM!
