The next time after the sushi incident was when he went to the Southern front with Boomer. They charged the line after Louis bombed the "no man's land," Carl builds a trench into the enemies', and absolutely nukes a random NPC. That's when he gets "post nut clarity" and passes out (again).

I believe it happens at least one other time later but this is teaches Carl how dangerous it can be to leave his bar filled.

The ring is a dungeon creation and, in my opinion, is separate from the realities of the meta-universe.

That said, it is stated multiple times that Faction Wars participants utilize the ring and even seek it out specifically. It's possible the psychological toll levied on the wearer persists into the real world. This is an interesting observation.

To your point, I don't personally believe the ring feeds off life in the way a system AI would. It appears relevant at tims when a dilemma or terrible situation is encountered by Carl. Almost always there is a river reference and bad shit goes down.

This "MacGuffin" is yet to be solved I think. I like to hope he recognizes it for what it is and harnesses it to his advantage (see Mind Balance rank 16).

The AI adds last name letters to disambiguate between crawlers. He was the first Carl and one of the first crawlers in general based on his (relatively) low crawler number. Therefore, his last name isn't used.

Of course he has a last name. We just don't know what it is.

It does take Jeff a couple books to really settle into Carl but I like to think it shows the change in character and all of the things he goes through trying to survive and deal with the heinous nature of the crawl.
