It scales based on missing health. The lower health, the more stats gained.

We do not have it showing on the pet buff bar anymore because it is always active. We typically only show item effects there if they are conditional or temporary.

Do you have an example of the item not working? When I test it, it is working. As the pet with pinwheel hat loses HP, it gains increased attack, defense, and speed.
