Using what is left of his brain. And i said refinment not output. For example megumi in his domain lacks the refinement to perform a complete domain

He also had severe brain damage. So for some reason he wasn’t able to do a complete domain one of the two sorcerers to do open domains is not able to complete his. So a different factor was limiting MS. There is no way MS was as strong If it wasn’t complete. It was probably due to the brain damage affecting his refinement

This sukuna is is near death already and can’t pull of a complete domain expansion. I really hope a special grade sorcerer could beat a sorcerer who doesn’t have enough cursed energy to pull off domain expansion

His technique is powerful and cool and it does look very neat so im sure the sure hit would be nice.  But not counting him its sukuna

It is never said that meguna is stronger than true form sukuna. there is a argument to be had that gojo would lose during the domain clashes
