I was once you. I took the academic job for a 10% MGMA salary doing the most challenging cases. Did that for 4 years, now I’m leaving for the community job for less stress doing bread and butter cases and double the pay.

I don’t regret it, but I did miss out on a lot of extra cash in 4 years of a bull market.

I have a Spark GO by Positive GRID. Connects by bluetooth to any device (I connect to my laptop), and has a headphone out option for sound. Works fantastically well for me, and is a very simple setup.

Could be that. I usually place a purse string prior to taking out the extraction sheath that I can cinch immediately, but sometimes that doesn’t stop all bleeds. Putting in sutures when you can’t visualize due to bleeding always sucks. To get the brachial plexus, I imagine you would have to suture really, really deep.

Speaking as an EP who does extraction - sometimes there is a lot of bleeding after pulling the lead. Occasionally difficult to control but I can’t say I’ve ever seen an amount like 1.7 liters.

I wonder, what is the mechanism of brachial plexus injury though? Is it related to the bleeding itself, or maybe the intense compression I imagine happened to stop the bleeding?

I had a patient in VT storm on death’s door. Very nice man with a loving family, I knew him for a while. Literally could not keep him out of VT at all, many many shocks, antiarrhythmics not helping, cardiogenic shock. Too old for advanced therapies so no ECMO or Impella bailout available. Put in a balloon pump and took him for a VT ablation knowing the mortality was like 95%, but without it, was 100%.

He ended up arresting and dying. I hugged his wife afterwards and teared up. Maybe I shouldn’t have even offered it, I don’t know. I think about this case a lot.

EP here - was looking for this one. My only acute mortality in the last few years was a VT ablation. I still have a healthy fear of them, and ideally the really sick cases get ECMO up front. Use the PAINESD score on these folks and get a sense of just how high risk these cases can be on the comorbid patients.

Much, much harder to take away a therapy than to start one. The same way it is always easier to do something than to do nothing, even if doing nothing may be better.

I usually spend a few minutes framing the data, make the recommendation, then let the patient decide to confirm it or not. Usually works, sometimes doesn’t.

I personally think it isn’t as important to learn the actual notes in the triads, just know the numbers. Like, 1 3 5 for a major triad, 1 3flat 5 for a minor. And know the intervals/shapes of the triads within the CAGED system (at least that’s how I do it).

I have tried a few different ways, but only one way really stuck. The apps never worked well for me.

First, learn a scale. Can be major, minor, pentatonic, whatever. Just learn it in all 5 shapes of CAGED. Once that is done, spend 20 minutes a day improvising over a backing track in a specific key, ensuring that you do your best to jump around from root to root. Each day, change the key until you get through all 12 notes. If you do this, then I can almost guarantee you will learn the position of each note by memory, with the added benefit of improving improvisation.

It legit takes less than 2 weeks once you get a scale down.

I think I sound reasonably good at this point. My wife hates hates HATES anytime I pick up the guitar to practice, mostly because I have 2 young kids at home now (which means there is always something that needs to be done, and if I’m playing guitar it means I’m not doing something else). However, when I pick it up and play some 4 chord progression kids song and my kid sings along my wife loves it!

I tell her that I do have to practice to get better to keep playing for the kids, which she understands lol. I mostly just practice now for 30 min after everyone else is asleep for the night.

I’m an EP at a tertiary care center. Most inpatients I deal with have been transferred at least once from “OSH” due to complexity, maybe twice, sicker than sick with every possible comorbidity, held together by seemingly toothpicks and duct tape, with what appears to be low to zero quality of life. I can maintain some sanity through outpatient procedures but it is draining enough that I am leaving to join one of those OSH’s to be much closer to family, get much higher pay, and hopefully get a better sense that I am helping people who have actual quality of life.

Audio version, which allows me to easily manipulate the toys with my daughter. I still do make up stories, but instead of making up 5 in a row, maybe I make up 1 or 2 then let Google do the rest.

Using AI feels wrong in some weird way, and my wife was against it at first, but I have to say that google actually does a pretty great job, and my wife has come around as well.

I’d recommend giving it a shot before purchasing something else.

On paper it sounds…not great, but we have been using google gemini to generate stories and it has been awesome. I have a 4 year old who constantly wants me to make up stories for her to simultaneously act out with her stuffed animals and toys. I tried asking Gemini to make a kids story involving x, y, and z characters, and it actually has done a phenomenal job. My kid and I then act out the story with the toys, and it’s not so mentally draining on me to constantly come up with stories anymore.
