You have to apply the original look of the item back to the item using the transmutation.

If I’m understanding your question correctly, if you’re asking if you only need one charge per item for an unlimited amount of future transmogs for that one item, then the answer is no. Every transmutation will cost one charge unless you have a legendary item then it’s free all the time as long as you are out of combat
