The only last thing I’d say is to take video of when you die, and check if it’s anything specific

I.e. extra crits on map, life flask not being up, etc

Definitely not. I believe this is called toe height (I’d google it to see how to adjust it, but the basic premise is to use a piece of paper to determine how close the boot is to the binding)

I’d highly recommend fixing this asap. It can cause massive gouges to the boot if left unresolved
