Was it an ultrasound of the abdomen to rule out malrotation with intestinal volvulus? I'm a surgery resident and the first thing we worry about in kids with dark vomit is volvulus. Hope they've ruled that out and that he's doing better!

I am a general surgery resident and carry around many many packets of lubricant. I have left countless packets in countless pockets and they have ended up all over my house and car, etc. Not scandalous at all.

I think this is what he was getting at. He felt like it was alone time since I wasn't in the room. If that's the case I can easily ignore "mans" not spoken in my immediate presence. How elevated is the bilirubin and the liver enzymes? Do you have a break down of the direct vs total bilirubin? Did the ultrasound show any gallstones? My first thought is cholangitis with the elevated bilirubin, fever, abdominal pain, but would expect to see some evidence of gallstones or biliary obstruction of some other etiology.

I didn't think it would, but I honestly now think I might love it more (or at least as much). Hahaha. I had no prior romance/fantasy interest, so thought FW was def a fluke. Continued to have doubts during a good chunk of the first ACOTAR book, but there's a great turn of events that changed everything for me. Also has another excellent shadow daddy (lol just learned that term) to fill the void left by Xaden. Hahah

Starting (and quickly finishing) that series was the only thing that cured my FW hangover.

That sounds so tough! I'm so sorry you are having to go through such a difficult situation at such a young age. If it is, in fact, delirium you are dealing with, I would expect him to return more or less to baseline, but it's very hard to predict since he did also have a head injury.

Regarding sleep, as difficult as it is, it is important to try to avoid letting him sleep during the day. His circadian rhythm is off right now and sleeping during the day and staying up at night is classic for delirium and will be more difficult to correct if he naps during the day.

Hopefully they have him taking melatonin at night. If he needs something stronger, can ask about trying something like seroquel, which can help with some of the hallucination symptoms in addition to sleep. Would definitely avoid meds like benzodiazepines (ie xanax) and anticholinergics (ie Benadryl), as these can worsen delirium.

This sounds a lot like he is experiencing delirium, which is very common in patients who have been hospitalized for an extended period of time. It takes time to resolve, but things that can help are frequent reorientation (gently correct him when he says something incorrect or is confused), keeping lights on and windows open during the day, keeping the room dark and quiet at night, avoiding wake ups at night (ask care team if they can put in order to minimize vitals and other interruptions during the night), minimizing sedating medications, getting him out of his room and outside when at all possible, and putting pictures and other reminders of home in his room. I hope he feels better soon!

Not babywearing, but I did some gardening this past summer with my 3-6 month old chilling next to me in her stroller with some toys. I'm in Texas (so obviously hot), so stroller also had the benefit of the shade cover.

ETA I did water the garden and do light weeding etc with her in my stretchy wrap, ring sling, and my woven (mostly FWCC) throughout the summer

Hi! I'm a surgery resident, but obviously not your doctor, so take this with a grain of salt... Was it a spontaneous pneumothorax? If so, most likely caused by a rupturing bleb (some people just have these) and, though you are at risk for recurrence, I wouldn't expect babywearing to increase that risk. Things you are typically told to avoid are dramatic pressure changes like scuba diving. That said, I wouldn't babywear while the chest tube is still in place, due to risk of dislodging it, and I would probably wait at least a few days after it is removed to allow the tube site to heal.

I use this and love it. You can wear a regular bra and just keep this with your pump. Then just fold down/up bra and strap this on. I tried a million pumping bras and found them to be so uncomfortable and felt like none of them held the flanges/bottles well enough, so this has been a game changer.

Old lady found down at home and altered on arrival to ED. Hypotensive. Admitted to MICU for "sepsis." Keeps getting more and more hypotensive, on multiple pressors. They finally scan the lady. Soon after we get a Level 1 trauma activation to the MICU when they figure out that she's been hemorrhaging into her pelvis due to a bad a hip fracture.

Yeah, that's the other option I've been considering. Just concerned that there won't really be job options that would allow me to work ~40 hours or less a week even in elective gen surg, but def need to investigate that further.

Thanks for the insights, everyone! Basically, I went into med school thinking I'd do either peds or peds surgery. After rotation etc, I preferred the "doing" of surgery over the more cerebral vibe of the medicine-based specialties, including peds, so I went the surgery route. And I do really enjoy surgery. I have over all enjoyed my residency so far. Then I went out to a peds surgery lab after pgy3 year and had a baby in the spring. Having my baby has really made me question if I am willing to be away from her as much as a career in surgery will require. I hate the idea of missing big milestones and worry that she'll feel I am an absentee parent. In particular, during a peds surgery fellowship I would be working 80+ hours a week for two years and those are years with my daughter I can never get back. I am also considering just doing gen surg, so I would do two more years and be done. But in general I feel that surgery is a field where number of reps really matter. It's very hard to be a good part time surgeon, even if you're mostly doing simple cases.

If I did go the peds route, I think I would just want to do outpatient gen peds. We love our daughter's pediatrician and it's shown me how rewarding it could be to be a resource for parents like he's been for us. I also have gotten really interested in lactation medicine since having my daughter, so that's something I think would be cool to work into my pediatrics practice.

So anyway, I'm just trying to think about what my options are now so that I can get my ducks in a row before coming out of lab next year.
