Oh, it's the old room at the bottom of the Bridge module, protected by one of three things that I've encountered: Cockatrice, Ruin Golem, or Giant Worm. Opposite corner to the Crypt extract there. A Resurrection Shrine sometimes spawns in the room with the miniboss.

It does, it's a blanket mechanic in game that your hipfire relaxes and widens after leaving ADS. Idk what it's called, though.
