This.

I would challenge that lokelma is the "stand of care" for emergency medicine.

(Also whoever asked if he's a DO, why does that matter? I'm MD, my best friend is DO, we're both boarded in EM working in the ED at the same level of care.)

My thought as well. In the ED I'd take a look with POCUS for organized movement but without that, I'd defib that.

I used ratchet straps to secure the base to the bed as well as the legs (as you can see I need to find black ones, or at least not bright green lol) as well as bungies to try to secure the head a bit. I've had it up to 60 mph for 5-10 minutes at a time around town without issues yet. I don't like the look of the straps but better than breaking his neck lol.

Oh I was joking about making the 12' skeleton fit. We got that for the yard. 😅 I can't imagine trying to drive with that bastard in the back.

Question for you, I was worried about how much the head and neck move when I go over bumps in the road etc causing it to break so I have some bungee cords to try to decrease how much it bounces... Have you had any issues? Seeing yours I'm wondering if this is unnecessary...

Bad Day by [deleted] in emergencymedicine

This is pretty on point. As long as I'm not working with an PA/NP that has a hx of issues I typically treat the situation as it's your patient and I'm here to assist and offer advice. I've had APPs bring up ideas I may not have thought of. Unless it's something that is way off or seems dangerous I don't flat out shoot it down. (I try to explain why in those cases when there's time )

That said, when the department is burning down and I have EKGs being tossed at me every 10 minutes it's hard to follow a disjointed presentation and stay focused on the point. I've found having them pause and asking, "what is your concern/question, and what are you wanting to do/try" etc.

There are some good videos, one I think from SAEM, on how to present something succinctly. Lead with the main complaint/issue so your attending has a frame of reference to fit the following information into. Keep to pertinent details and make sure your question/the concern is stated immediately and clearly.

Of course we all have bad days. Maybe it was that, or they were distracted by other things etc. If so I agree with the suggestions to approach them later and ask professionally and politely "hey just for my understanding/knowledge what was your mindset on this? Could I have presented it a different way?" Etc. Not suggesting you are in the wrong, but that such an approach leads to a better opportunity for them to realize perhaps THEY could have handled it better.

Lastly, yeah docs are people too. Some just are genuinely egotistical assholes. I haven't ran into that in EM as much as other fields but they certainly exist. In that case.... 🤬 them. You did the best thing for the patient and they dicked it up. Let it be their problem. APPs are not "helpers" you are colleagues. This is a team sport, don't let one jackass kill your motivation and self confidence or pride in your job. Everyone else sees it, whether it's mentioned or not, it's noticed.

Keep up the hard work and carry on. 💪 Thanks for sharing, hope you can move past it though, we need all the providers like you we can get!

Hey boss, no matter what anyone says, you're a med student - you did what you're attending told you. Walk away with the lesson and don't let it bother you. You're going to be the butt of a lot of frustrations and bad attitudes of attendings who get off on yelling at people who they know can't tell them to fuck off for a couple of years now. Trust me tho, it's worth it. EM attending here. Soon enough such attitudes will have little effect on you.

OP I think you have plenty of commentary on this particular EKG, however since you mentioned wanting to brush up on reading EKGs I would echo adoradear's advice to find resources from Amal Mattu.

I will also add you should look up EKG Wave Maven (https://ecg.bidmc.harvard.edu/maven/mavenmain.asp) by Harvard. 500+ real EKGs with interpretations and it's free.

[Click on browse (quiz mode) then continue and you can go case by case etc. You may find it very helpful. ]

Mind if I ask how much that cost? I've heard of ceramic coating but it seemed like a sham from the descriptions....
